Prostatic capsular incision during radical prostatectomy has important oncological implications: a systematic review and meta‐analysis

Abstract: The absolute impact of CapI on oncological outcomes is currently unclear due to inconsistent definitions. However, the data implies an association between CapI and PSM and BCR. Reporting of possible areas of CapI on the operation note, or marking areas of concern on the specimen, are critical to assist CapI recognition by the pathologist. “…Erectile dysfunction is a common and significant side effect of radical prostatectomy (RP). Nerve’sparing (NS) RP improves recovery of erectile function [1–3]; however, the quality of NS is often balanced against the oncological risks associated with positive surgical margins (PSMs) [4,5]. Clinical suspicion of extraprostatic extension of prostate cancer is a relative contraindication of NS RP [6] as it confers an increased risk of PSMs; however, accurate prediction of prostate cancer stage T3 (i.e.…”

“…The surgeon may have also concerns about the safety of the dissection in an imaging‐safe area. Inadvertent incision into glandular tissue may occur and awareness of persistent benign prostate tissue at the margin of dissection is also important .…”

“…However, the experience and skill of surgeons as well as an approach such as open RP versus minimally invasive RP (laparoscopic RP and robot-assisted RP) are known to affect the PSM rate and location of PSM. 5,12 Therefore, the data on these points are important to precisely interpret the result. In addition, long-term studies on the impact of location of PSM in nerve-sparing RP are needed, not only for BCR but also for other outcomes such as metastasis-free survival, cancer-specific survival, and overall survival.…”
