Objective: Croup due to infection with the omicron variant of COVID is an emerging clinical entity, but distinguishing features of omicron croup have not yet been characterized. We designed a study to compare the clinical features of croup patients presenting to the pediatric emergency department pre-COVID pandemic with COVID-positive croup patients who presented during the initial omicron surge.Methods: This was a retrospective observational cohort study of children 0 to 18 years old who presented to our urban, tertiary care pediatric emergency department with symptoms of croup. The study compared a cohort of croup patients who presented in the year before the onset of the COVID pandemic to a cohort of COVID-positive croup patients who presented during the initial omicron surge. The primary outcomes included illness severity and treatments required in the emergency department. The secondary outcome was hospital admission rate.Results: There were 499 patients enrolled in the study, 88 in the omicron croup cohort and 411 in the classic croup cohort. Compared with the classic croup patients, omicron croup patients were more likely to present with stridor at rest (45.4% vs 31.4%; odds ratio [OR], 1.82; confidence interval [CI], 1.14-2.91) and hypoxia (3.4% vs 0.5%; OR, 7.22; CI,. Omicron croup patients required repeat dosing of inhaled epinephrine in the emergency department more often (20.4% vs 6.8%; OR, 3.51; CI, 1.85-6.70), and they were more likely to require respiratory support (9.1% vs 1.0%; OR, 10.18; CI, 2.99-34.60). Desperate times demand optimistic transdisciplinary measures. This volume unites a select group of thinkers who courageously traverse disciplinary boundaries. What brings them together is the least stratified ‘component’: a shared problem. It is a widely recognised that a problem gets the solution it merits. However, only a few acknowledge that a problem seldom neatly fits within a single discipline, nor does it conform to the principle of general equivalence. Handling its irreducibility and non-entailment is a skill possessed by very few. Even fewer take the quasi-causal capacity of what we term the ‘space of technicity’ seriously. The space of technicity, the shared problem of this volume, is a consequence of immanence. Each configuration of surfaces comprising the built environment produces an intangible effect, acting as a quasi-cause. It can be referred to as downward causation or the timely rediscovery of (neo)finalism. In this volume it is approached it from the perspective of axiology. The space of technicity allows us to evade techno-determinism without adopting an anything-goes attitude. That which has become manifest could have individuated differently. However, the potential of a body cannot be discerned before intervening in the causal fabric of agential reality to extract the singular points that make certain outcomes more likely than others, surpassing mere probability. When operating within the ethico-aesthetic paradigm, where sense becomes intricately dependent on sensibility, and vice versa, the volume’s attitude might be said to approximate the Spinozian third kind of knowledge that intuits design (and its space of technicity) beyond mere imagination or reason. Bacterial tracheitis (BT) is an uncommon life-threatening condition that results in acute upper airway obstruction. Classical signs include a toxic appearance, stridor, tachypnoea, and fever, often leading to rapid clinical deterioration. Recent studies have shown a shift in BT epidemiology and presentation, where stridor and respiratory distress are now predominant. A poor response to corticosteroids or nebulized epinephrine is also commonly described, along with a need for mechanical ventilation. We present the case of a five-year-old boy admitted to the emergency department with cough, stridor, and dyspnea that had significantly worsened over the previous hours. He presented reasonable general condition, marked retractions, poor air entry, stridor, and wheezing. Investigation revealed a slight elevation of C-reactive protein and leukocytosis with neutrophilia. Anteroposterior x-ray showed narrowing of subglottic airways (steeple sign). There was no response to oral/nebulized corticoids, nebulized adrenaline, or bag-valve-mask oxygenation. Antibiotic therapy with ceftriaxone was initiated. Due to deteriorating clinical conditions with severe respiratory acidosis, orotracheal intubation was required. Later Streptococcus pyogenes was isolated in the bronchial secretions and a targeted antibiotic regimen was administered. Progressive clinical and analytical improvement was observed with no complications. Although uncommon, BT remains a severe infectious condition affecting otherwise healthy children. Our case underscores the severity of the disease and the imperative for invasive interventions to achieve favorable outcomes. It also supports recent findings indicating a shift in predominant symptoms and prognosis. Clinicians must be vigilant and knowledgeable, recognizing that worsening stridor and respiratory distress unresponsive to conservative treatment are key indicators for diagnosing BT.
