Protective effect of LASIK flap in penetrating keratoplasty following blunt trauma

Abstract: Penetrating keratoplasty (PKP) often results in large and unpredictable refractive errors following suture removal in the postoperative period. Laser in situ keratomileusis (LASIK) is an effective means of correcting these errors. However, LASIK following PKP is believed to further weaken an already weak graft-host junction and may predispose such eyes to traumatic dehiscence of the graft-host junction. We describe a case in which the LASIK surgery following PKP seemed to benefit the patient by preventing comp… Show more

“…Its popularity stems from its ability to correct a wide range of refractive errors with reportedly better and more predictable visual recovery outcomes than other procedures [2, 68]. Complications are relatively low in incidence but include corneal perforation and scarring, flap-related issues, epithelial ingrowth, and graft-host junction dehiscence [2, 8, 9]. Taken together, femto-LASIK has become a reasonable option for correcting residual refractive errors from PK.…”

“…The difficulties in LASIK include challenging preoperative evaluation, irregular corneal surface, previous AK cuts, corneal biomechanics change and inadequate stromal thickness for full HOA correction [48,52 & ,96,97]. The resultant complications can be flap-related ones, increased risk of graft-host junction dehiscence, epithelial ingrowth, and corneal scarring [7,91,98].…”
