Protective Factors in Posterior Urethral Valves in Saudi Children

Abstract: Posterior urethral valves are the most common cause of bladder obstruction in boys. Clinical presentation may vary, depending upon the degree of obstruction and its sequelae. Pathological changes range from relatively normal upper tracts or a small amount of dilatation to a massive dilatation associated with severe infection or sepsis, azotemia and/or renal dysplasia. In our population they constitute about 17% of chronic renal failure in children. 1The development of renal insufficiency in patients with poste… Show more

“…Over a decade ago, Rittenberg et al [5] reported some protective factors that may lead to improved prognosis in children with PUV. More recently we observed the beneficial effects of such protective factors as unilateral vesicoureteral reflux with ipsilateral renal dysplasia, urinary ascites, urinoma, or large congenital bladder diverticula among our patients [6]. There is still controversy regarding the optimal initial therapy for children with PUV.…”

“…The formation of urinomas and urine ascites protects the kidney from the consequences of chronic obstruction by allowing relative decompression of the pelvicalyceal structures. Other means of relieving the high pressure within the collecting sys‐ tem is the formation of bladder diverticula, and vesico‐ureteric reflux 11 . In patients with posterior urethral valves, 50% have reflux at presentation.…”
