Protein contact dermatitis: myth or reality?

Abstract: Protein contact dermatitis is a dermatosis which usually presents as a chronic eczema with episodic acute exacerbations a few minutes after contact with the offending allergen. Patch tests with the responsible allergen are usually negative, and the diagnosis can only be made by means of scratch or prick tests with the allergen. Sometimes, specific IgE antibodies can be detected in the blood. As there is considerable confusion about this entity, we have reviewed the cases reported in the literature.

“…The substance can penetrate via the SC, including the corneocytes and the lipid layers, or via the follicles. Skin reactions were observed after contact with substances containing proteins up to a molecular weight of 75 kDa [16,51,52] . Bos and Meinardi [53] argue that the molecular weight of a compound must be below 0.5 kDa to allow skin absorption in healthy skin, whereas for atopic skin, also molecules of slightly over 0.8 kDa can be absorbed.…”

“…Gelegentlich treten auch kurz nach dem direkten Hautkontakt mit Proteinen akute urtikarielle oder vesikulöse Hautveränderungen auf. Durch eine häufige Reexposition im Rahmen einer beruflichen Tätigkeit sind die Läsionen meist an Händen und Unterarmen lokalisiert [3,5,9].…”
