Proteinase 3 and dihydrolipoamide dehydrogenase (E3) are major autoantigens in hepatitis C virus (HCV) infection

Abstract: SUMMARY Hepatitis C virus (HCV) infection has been found to be strikingly associated with autoimmune phenomena. The aim of the present study was to investigate the presence of various autoantibodies in patients with HCV infection. Anti-neutrophil cytoplamic antibody (ANCA), anti-dihydrolipoamide dehydrogenase (anti-E3), rheumatoid factor (RF), anti-dihydrolipoamide acetyltransferase (anti-E2), anti-SS-A/Ro (60 kD), anti-SS-A/Ro (52 kD), anti-SS-B/La, anti-topoisomerase II (anti-topo II), anti-ca… Show more

“…One study found two P-/MPO-ANCA-positive individuals among 22 individuals who were infected with hepatitis B virus (157). Similar reports have emerged regarding the presence of ANCA in patients with hepatitis C infection (158,159), including one study that found ANCA in as many as 56% of patients with a predominant anti-PR3 specificity (158). Although the high prevalence of ANCA in the setting of hepatitis is intriguing, evidence for a causal association between these viruses and ANCA-associated vasculitis remains poor.…”

“…In the present study, ANCA was present in 18 of the 136 patients (13.2%), with higher prevalence in cryoglobulin-positive than cryoglobulinnegative patients (25.5% vs 5.9%, OR 5.4, 95% CI 1.9-16.4). HCV patients with ANCA had a higher prevalence of skin involvement, anemia, abnormal liver function tests and elevated alpha feto-protein levels [75] . Anticardiolipin antibodies have been observed in 3.3% to 22% of HCV patients [77][78][79] .…”

“…Recent data suggests that the HCV core particles concentrated in the cryoprecipitate play a role in the interaction between the cryoglobulins, endothelial cells and neutrophil granulocytes [74] . The main antigens in HCV patients with a positive ANCA test are proteinase 3 and dihydrolipoamine dehydrogenase [75] . A positive ANCA has been found in 10% patients with HCV-associated mixed cryoglobulinemia [76] .…”

“…Although PR3-ANCAs may occasionally occur in other inflammatory conditions including certain infections, [11][12][13][14] they are highly specific for WG. 15 Many in vitro experiments suggest a pathogenic role of PR3-specific autoantibodies; nevertheless, their in vivo relevance has been disputed since their discovery.…”

“…PR3-ANCAs have been reported to occur transiently in a variety of infections, but the development of systemic vasculitis in these patients remains an exception. [11][12][13] Cocaine abusers may also develop ANCAs, some of which react with PR3. 36 Although systemic vasculitis or glomerulonephritis was not observed in these patients, the clinical features prompting them to seek medical attention were their dramatic localized inflammatory tissue destruction.…”
