“…Hemodilution during pediatric cardiopulmonary bypass (CPB) universally affects all components of the coagulation cascade in unpredictable proportions resulting in a significant coagulopathy . Neonates and infants have relatively high concentrations of plasma fibrinogen and the large hemodilution consequent to bypass in this age group makes fibrinogen the first clotting factor to decrease to critically low levels .…”

“…Nevertheless, administration of these products is plagued with thrombotic risks, which could be life-threatening following intricate cardiac surgery. 9 Based on this case study, experience and the aforementioned literature, two modifications to institutional protocols are recommended. The first is the use of CVVH in theatre to eliminate bivalirudin in renal failure patients or in patients where bivalirudin elimination is prolonged.…”

“…This report also highlights the need for the development of bivalirudin protocols to include sections for management of post-CPB refractory bleeding. As refractory bleeding post cardiac surgery occurs in 10%-15% of patients 9 and one of the major concerns when using bivalirudin is the increased risk of post-cardiac surgery bleeding due to the lack of a reversal agent. With this knowledge, no study or trial has outlined a protocol for the management of refractory bleeding post-CPB with bivalirudin anticoagulation.…”
