Aim: This research was conducted to assess the relationship between maxillary sinus floor and the maxillary posterior root tips position in a Sample of Egyptian Population using CBCT. Subjects and methods: This cross-sectional study evaluated 144 maxillary premolars and 144 maxillary molars from 41 CBCT scans of 19 male and 22 female patients according to certain eligibility criteria. Image analysis was performed on axial, corrected coronal and corrected sagittal images using Planmeca Romexis® Viewer software. The vertical relationship between maxillary sinuses and maxillary premolars was assessed according to Razumova et al., 2019 classification, while the vertical relationship between maxillary sinuses and maxillary molars was assessed according to Kwak et al., 2004 classification. Inter and intra observer agreement were done using Kappa statistics. Results: Most common vertical relationship between all maxillary premolars and maxillary sinus was Type I while most common vertical relationship for all maxillary molars was Type II. The largest distance was found in first premolars' buccal roots while the smallest distance was found in second molars' mesio-buccal roots. There was a significant inverse correlation between age and vertical relationship classes of only 2 nd molar roots. Conclusion: Type I vertical relationship was the most common relationship between all maxillary premolars and MSF while Type II vertical relationship was the most common relationship for all maxillary molars. The largest distance was found in first premolars' buccal roots while the smallest distance was found in second molars' mesio-buccal roots. The lower the age is the closer are the maxillary 2 nd molar roots to the MSF.

Background The proximity of the maxillary sinus floor to the maxillary molar roots increases the probability of oroantral communication on conducting any surgical or endodontic procedure in the involved area. The aim of this study is to evaluate the relationship between each maxillary molar root and maxillary sinus floor using cone beam computed tomography. Predicting the probability of protrusion of each root into the sinus will consequently predict the probability of occurrence of the oroantral fistula in a sample of the Egyptian population. Results The total number of roots located outside the sinus was 121 (35.3%), while those contacting the sinus floor were 80 (23.3%) and those intruded the sinus were 141 (41.2%). The percentage of root intrusion into the sinus in males (56.9%) was significantly (p = 0.01) higher than females (42.9%). The probability of root intrusion in the left molars (54.2%) was non-significantly (p = 0.067) higher than that of the right side (44.3%). As for the type of tooth, the second molar showed the highest probability of root intrusion into the sinus (55.3%) followed by the third molars (52.6%) then the first molars (40.9). According to the type of root, the mesiobuccal root showed the highest probability of intrusion into the sinus (50.9%) followed by the palatal root (49.1%) then the distobuccal root (47.4%). However, the difference in both type of tooth and type of root was statistically non-significant (p = 0.051 and 0.869 respectively). As for the individual root with the highest probability of intrusion, the mesio-buccal root of the right third molar is the most frequent root to intrude the sinus (71.4%) and the mesio-buccal root of the right first molar is the least frequent (22.7%). Conclusions In a sample of the Egyptian population, males exhibit higher probability of root protrusion into the sinus than females. The side and type of tooth are of higher impact on the probability of its intrusion into the sinus compared to the type of root. Left second molars are at a higher risk of oroantral communications on surgical or endodontic procedures compared to other molars due to its highest probability of intrusion into the sinus.

Aim:To assess the proximity between the roots of maxillary molars and Maxillary Sinus Floor 'MSF' using CBCT in a sample from the Egyptian population. Materials and Methods: Eighty seven CBCT scans of adult Egyptians (29 males and 58 females) involving 135 maxillary 1 st molars, 168 maxillary 2 nd molars and 107 maxillary 3 rd molars, were collected from patients' data base of Planmeca Promax 3D Mid CBCT machine available at the Department of Oral &Maxillofacial Radiology, Faculty of Dentistry, Cairo University. The vertical relation between MSF and examined teeth roots was assessed on cross sectional CBCT images based on Jung classification 2009Results: In all mesio-buccal "MB', disto-buccal 'DB'& palatal 'P' roots of the examined teeth, the 1 st molars showed the highest prevalence of class (0) in the 3 roots, while the 2 nd molars showed the highest prevalence of class (3) among both MB & DB roots, and the highest prevalence of class (2) among P roots, finally 3 rd molars showed the highest prevalence of class (1) among DB roots, and the highest prevalence of class (3) among P roots. There was a significant inverse correlation between age and vertical relationship classes of only the first maxillary molars roots. Males showed higher prevalence of Class (2) and Class (3) while females showed higher prevalence of Class (0) and Class (1) in the 3 rd molars. Conclusion:In the investigated Egyptian subjects, maxillary 1 st molars' roots are the least close to the MSF compared to the other molars roots, while buccal roots of maxillary 2 nd molars are highly anticipated for MSF invagination. The lower the age is, the closer are the maxillary 1 st molar roots to the MSF, and males' maxillary 3 rd molars are more frequently approximating MSF than females.
