Pseudogout: An Autoimmune Paraneoplastic Manifestation of Myelodysplastic Syndrome

Abstract: Myelodysplastic syndrome (MDS) is often associated with autoimmune paraneoplastic manifestations. Seronegative arthritis is among one of them. Very rarely, pseudogout demonstrated as paraneoplastic autoimmune manifestations of MDS has been adumbrated so far. Our case would be the another addition in the series. Our patient is an 83-year-old male lately diagnosed with MDS. After six months of initial diagnosis, he had a sudden onset episode of pain and swelling involving left wrist. Synovial fluid analysis from… Show more

“…Tedeschi et al [29] and Iqbal et al [30] were the only authors to report a case of paraneoplastic arthritis mimicking pseudogout correlated with an underlying myelodysplastic syndrome. Lima et al [31] described the case of a paraneoplastic polyarthritis, polymyositis and hypercalcemia developing in a patient an acute lymphoblastic leukemia on a chronic myeloid leukemia (CML).…”

“…CKD was defined as present if it was included on the problem list or clinical notes within 30 days of the index date and encompassed a range of severity, and it is possible that different stages of CKD may be associated with different risks for acute CPP crystal arthritis. Recurrent acute CPP crystal arthritis has been noted in case reports of patients with myelodysplastic syndrome (MDS) and/or acute myelogenous leukemia (AML) who have not received granulocyte colony stimulating factor (the latter of which has been linked with acute CPP crystal arthritis flares) 14,15,16 . The pathophysiology linking acute CPP crystal arthritis and MDS/AML remains unclear; it is possible that systemic inflammation related to neoplasm is a triggering factor.…”

“…CPPD is an idiopathic condition, however, it has a predilection to affect patients with certain risk factors. Aging, previous joint injury, osteoarthritis, hyperparathyroidism, hypermagnesemia, hypothyroidism, and hemochromatosis are some of these risk factors [4,5]. Chronic cases of CPPD may mimic rheumatoid arthritis or osteoarthritis [6].…”
