Psychogenic psychoses A RETROSPECTIVE STUDY WITH SPECIAL REFERENCE TO CLINICAL COURSE AND PROGNOSIS

Abstract: The study comprises a retrospective evaluation of case records of 220 patients admitted for the first time with psychogenic psychosis with special reference to clinical course and prognosis within a period of 13--14 years. The reliability of the diagnosis at the time of the first admission is about 60% when the evaluation is based on positive criteria for psychosis. “…Stromgren stated that the excitative and depressive subtypes of reactive psychosis are the most common, accounting for 65% of cases, with the depressive type predominating (20). Andersen & Laereum reported that 79% of patients with reactive psychoses presented with depressive symptoms, and that those with excitation and elation were more likely to be suffering from a manic-depressive disorder (21). Dunner emphasized that the presence of manic symptoms compels the clinician to favour a diagnosis of bipolar affective disorder (22).…”

“…From 1945 to 1954, reactive psychoses made up 4% of all admissions for patients over 60 to Arhus Psychiatric Hospital in Denmark, all but one occurring in the 60-to 69-year age bracket (35). Approximately 7-142 of all patients with the diagnosis of reactive psychosis were over the age of 60 (9,35). The absolute number of elderly psychiatric patients hospitalized with the diagnosis of reactive conditions shows an increasing trend with a clear female predominance (36).…”

“…This was highest in young patients and in the first few years following the psychotic episode. Accordingly, in some other studies excess mortality in patients with reactive psychosis has been suggested (Andersen and Laerum, 1980). Odegaard, however, reported lower relative mortality in patients with reactive psychosis than with schizophrenia or manic-depressive psychosis (Odegaard, 1967).…”
