IntroductionTranscranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that modulates brain states by applying a weak electrical current to the brain cortex. Several studies have shown that anodal stimulation of the ipsilesional primary motor cortex (M1) may promote motor recovery of the affected upper limb in patients with stroke; however, a high-level clinical recommendation cannot be drawn in view of inconsistent findings. A priming brain stimulation protocol has been proposed to induce stable modulatory effects, in which an inhibitory stimulation is applied prior to excitatory stimulation to a brain area. Our recent work showed that priming theta burst magnetic stimulation demonstrated superior effects in improving upper limb motor function and neurophysiological outcomes. However, it remains unknown whether pairing a session of cathodal tDCS with a session of anodal tDCS will also capitalise on its therapeutic effects.Methods and analysisThis will be a two-arm double-blind randomised controlled trial involving 134 patients 1–6 months after stroke onset. Eligible participants will be randomly allocated to receive 10 sessions of priming tDCS+robotic training, or 10 sessions of non-priming tDCS+robotic training for 2 weeks. The primary outcome is the Fugl-Meyer Assessment-upper extremity, and the secondary outcomes are the Wolf Motor Function Test and Modified Barthel Index. The motor-evoked potentials, regional oxyhaemoglobin level and resting-state functional connectivity between the bilateral M1 will be acquired and analysed to investigate the effects of priming tDCS on neuroplasticity.Ethics and disseminationThe study has been approved by the Research Ethics Committee of the Shanghai Yangzhi Rehabilitation Center (reference number: Yangzhi2023-022) and will be conducted in accordance with the Declaration of Helsinki of 1964, as revised in 2013.Trial registration numberChiCTR2300074681.

Background Most stroke survivors have upper extremity dysfunction. According to neuroplasticity theory, transcranial direct current stimulation (tDCS) and kinesthetic motor imagery-based brain computer interface (KI-BCI) have the potential to improve the upper extremity function of participants with subacute stroke. However, the efficacy of tDCS combined with KI-BCI in participants with subacute stroke is unknown. Objective To investigate whether the combined effect of tDCS and KI-BCI on upper limb function in participants with subacute stroke is more effective than the effects of tDCS or KI-BCI alone. Methods We randomized 48 participants into a tDCS group (n = 16), a KI-BCI group (n = 16), and a tDCS-BCI group (n = 16). Participants in the tDCS group received 30 min of tDCS with the anode over M1. The KI-BCI group performed 30 min of KI-BCI training. Participants in the tDCS-BCI group received 15 min of tDCS and 15 min of KI-BCI. All participants received conventional intervention. The treatment cycle consisted of a 1 session each day, 5 days per week for 4 weeks. The Fugl–Meyer Assessment of Upper Extremity (FMA-UE) subscale, Motor Status Scale (MSS), Action Research Arm Test (ARAT), and Modified Barthel Index (MBI) were used to assess upper limb function, and activities of daily living (ADL) before and after the 4-week treatment period. In addition, electroencephalography (EEG) was used to explore potential clinical brain mechanisms. Results After four weeks of intervention, the tDCS-BCI group was superior to the tDCS group in terms of the MSS. The FMA-UE, MSS, and MBI scores of the KI-BCI group were superior to those of the tDCS group. There was no difference in the number of quantitative EEGs among the three groups, while the number of quantitative EEGs was greater than before. Conclusion TDCS combined with KI-CI training can improve upper extremity function. However, KI-BCI training alone can improve upper limb function and ADL simultaneously. TDCS could alter the electrical excitatory levels of the cerebral hemispheres. Trial registry number: ChiCTR2000034730

The upper extremity functional index (UEFI) is a commonly used patient-reported outcome measure with good measurement properties. The Arabic UEFI has been reported to have sufficient reliability and construct validity, but its structural validity has not been examined yet. The aim of this study was to examine the structural validity of the Arabic UEFI using both exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) in patients with upper extremity musculoskeletal disorders. Participants with upper extremity musculoskeletal disorders attending physical therapy clinics were recruited. Participants were asked at their initial visit to physical therapy to complete the Arabic versions of the UEFI, and the numeric pain rating scale. In the EFA, the number of factors was determined using parallel analysis and maximum likelihood was used for extraction. The fit of the structural model identified using EFA was examined using CFA with multiple fit indices. The results of the EFA suggested one factor structure accounting for 57.22% of the total variance. All the UEFI items showed substantial loadings on the single factor that ranged from 0.51 to 0.85. The CFA multiple fit indices did not support the fit of the unidimensional structure at the first run of the analysis. Adding error covariance to items of similar function led to good fit of the data to the modified unidimensional model; χ2 = 665.94 (df = 162, P < 0.001), χ2/df = 2.88, Tucker–Lewis index (TLI) = 0.91, comparative fit index (CFI) = 0.92, root mean square error of approximation (RMSEA) = 0.086 (90% CI = 0.077-0.095), and standardized root mean residual (SRMR) = 0.045. The results of the EFA supported this unidimensional structure of the UEFI while the CFA supported this unidimensional structure after accounting for possible response dependency among some of the items. Clinicians and researchers can use one total score for the Arabic UEFI that reflects the level of upper extremity function.
