Background: The family caregiving process requires adaptation and be evaluated to ensure a correct approach in supporting patients and their caregivers. The objective of the present study is to design and validate an instrument to determine the family caregivers’ adaptation to the caregiving process in relation to the care dependency of hospitalized patients. This paper presents CUIDARE, a validated scale based on the Nursing Outcomes Classification indicators and the Roy Adaptation Model, that evaluates the adaptation process of caregivers in hospital settings and its impact on hospitalized patients. Methods: A cross-sectional and psychometric validation study was carried out in ten public hospitals in the Andalusian Healthcare System. Data collection was undertaken between May 2016 and February 2017 in medical and surgical units, included 432 family caregivers. Results: The CUIDARE scale was shown to be internally consistent (Cronbach’s alpha = 0.818). The Exploratory Factor Analysis grouped 4 items into a single component with a variance-explanatory power of 65.6%. The Confirmatory Factor Analysis tested the unidimensionality of the scale with good fit model (CMIN/DF = 2.152; AGFI = 0.975; NFI = 0.993; CFI = 0.996; RFI = 0.979; RMSEA = 0.052). Conclusions: There is an association between the patients’ care dependency level and the family caregiver’s adaptation to caregiving (p-value <.001). Nursing professionals are encouraged to apply this instrument for the purpose of screening to facilitate the adaptation process of family caregivers and to improve the quality of care.

Resumo Objetivos selecionar os indicadores dos resultados de enfermagem Integridade tissular: pele e mucosas (1101) e Cicatrização de feridas: segunda intenção (1103) da Nursing Outcomes Classification e construir suas definições conceituais e operacionais para a avaliação de pacientes com lesão por pressão. Métodos estudo de consenso de especialistas realizado em hospital universitário em setembro/2018. Participaram no estudo 10 enfermeiros com experiência na utilização da Nursing Outcomes Classification e no cuidado ao paciente com lesão por pressão. A coleta de dados ocorreu por meio de encontro presencial com os especialistas. Resultados Foram selecionados 17 indicadores da Nursing Outcomes Classification para a avaliação do paciente com lesão por pressão, com uma concordância de 100% entre os especialistas. São eles: Branqueamento, Eritema, Sensibilidade, Perfusão tissular, Hidratação/ Descamação, Espessura, Necrose, Odor desagradável na ferida, Pele com bolhas, Pele macerada, Descolamento Sob as bordas da Ferida, Inflamação Da Ferida, Exsudato/Drenagem, Granulação, Tunelamento, Formação de cicatriz e Tamanho da ferida. Conclusão e implicações para a prática os indicadores selecionados permitiram a elaboração de um instrumento que auxiliará na avaliação de pacientes com lesão por pressão de forma acurada. Esse instrumento subsidiará o enfermeiro na tomada de decisão diagnóstica e terapêutica da lesão por pressão.

PurposeAssessment of pressure ulcers is an indicator of nursing care quality. In physically handicapped patients, the risk is elevated, due to the neuropathy and incontinence that frequently accompany paresis and paralysis. We conducted a systematic review of 4 assessment scales to assess their use and usefulness for these patients.Materials/methodsWe searched 5 databases for articles published between 2016 and 2018 that included terms related to the risk of pressure ulcers in physically handicapped patients, including the names of 4 assessment scales (Norton, Braden, Waterlow and Spinal Cord Injury Pressure Ulcer Scale). One hundred and three sources were returned, from which we selected 10. We also selected an earlier source, for a total of 11.ResultsThe Braden and Waterlow scales are popular in clinical practice. The Norton scale is used for scientific comparisons and in clinical practice only sporadically. Expert opinion suggests that acceptance of the Spinal Cord Injury Pressure Ulcer Scale could reduce the incidence of pressure ulcers in physically handicapped patients.ConclusionsIntroduction of a scale designed for handicapped patients, combined with better preventive measures, could result in the bedsore reduction of pressure ulcers. Research testing the effectiveness of the Spinal Cord Injury Pressure Ulcer Scale in the Czech population is needed to validate its use in clinical practice.
