Puerperal ovarian vein thrombosis: evaluation with CT, US, and MR imaging.

Abstract: Five women suspected of having puerperal ovarian vein thrombosis (POVT) were examined with either computed tomography (CT) (five patients), ultrasonography (US) (three patients), magnetic resonance (MR) imaging (one patient), or a combination of these modalities. CT findings included tubular retroperitoneal mass (five patients), periuterine mass (four patients), enlarged uterus (four patients), fluid in the uterus (five patients), central ovarian vein thrombus (four patients), inferior vena cava thrombus (one … Show more

“…Ovarian vein thrombophlebitis is associated at surgery with a congestive ovary (10). However, US, CT, and MR reports in the literature emphasize direct visualization of the thrombus and do not describe the ovaries (11,12). No risk factor or clinical or imaging findings suggestive of ovarian thrombophlebitis were present in any of our cases.…”

“…Calderwood et al 3 stated that the right ovarian vein gets easily compressed due to the dextrorotation of the uterus and also due to its entry into the IVC at an acute angle. Savander et al 4 concluded that the right ovarian vein is longer than the left and has many valves within its length, which increases the risk of thrombus formation. Literature 5 also opines that the right ovarian vein is predisposed to thrombosis because there is an anterograde flow in the right ovarian vein, while a retrograde flow is noted in the left ovarian vein immediately after delivery.…”

“…5 Ayrıca, gebelikte uterusun dekstropozisyonuna bağlı bu bölgeye olan daha fazla bası ve buradaki venlerde kan akımının farklı özellik göstermesinden ötürü OVT olgularının % 80-90'ında patolojik olay sağ ovarian vende görülür. [4][5][6] Bizim hastamızda da SPT'ye yatkınlık oluşturacak herhangi bir risk faktörü mevcut değildi ve patoloji sağ ovarian vende gelişmişti.…”
