Pulmonary Complications Following Major Head and Neck Surgery With Tracheostomy

Abstract: Objective:To test the hypothesis that extended postoperative antibiotic cover would reduce the incidence of pulmonary complications in patients undergoing major head and neck surgery with tracheostomy.Design: A prospective, randomized, controlled trial was carried out to determine the efficacy of an extended course (5 days) of intravenous amoxicillin-clavulanic acid in reducing the rate of atelectasis and pulmonary infections postoperatively. Other possible risk factors that might predispose to pulmonary compl… Show more

“…The DMA instrument manufacturers do suggest sample dimension ranges for given geometries , but do not discuss how a three order of magnitude modulus change through the glass transition can affect DMA measurements. Numerous researchers favor the 3pt bend geometry , other favor the DC geometry , and others favor the SC geometry . However, Duncan and others claim that the 3pt bend geometry is best suited for high modulus materials, such as metals, ceramics, and highly filled thermosetting polymers that show little change in modulus throughout the test .…”

“…Complications of tracheostomy include haemorrhage, obstruction, displacement, local infection, pulmonary infection, fistulae, tracheal stenosis and stomal tumour recurrence 5,9 . It is uncertain whether these risks are justified for many patients undergoing major head and neck surgery.…”

“…Because the demands of long-term postoperative airway maintenance can vary depending on the patient's circumstances, elective tracheostomy is recommended for patients with a high risk of airway obstruction. However, careful patient selection is required in elective tracheostomy because complications such as bleeding, local infection, pneumonia, obstruction or airway distortion, and airway stenosis can occur after a tracheostomy is performed7,8,9. Moreover, because of the invasive nature of the tracheostomy, this procedure can cause additional risks for the patient.…”
