Pulmonary embolism and deep vein thrombosis related to oral contraceptive use

Abstract: Pulmonary embolism (PE) and deep vein thrombosis (DVT) is a serious medical problem causing considerable morbidity and mortality. Risk factors of thrombosis are surgery, trauma, pregnancy, tumor, oral contraceptive as well as genetic risk factors though genetic risk factors were found in about 5% to 10% of cases. Yasmin, a combined oral contraceptive containing ethinylestradiol 30 µg and drospirenon 3 mg was launched in the United Kingdom in 2002. “…The link between OCCP containing estrogen and DVT is well established in the literature [ 13 ]. However, recent studies have illustrated that the addition of progestin contributes to an increased risk of DVT, but not if it was used alone, as it was shown by a systematic review [ 14 , 15 ]. One study shows that overall, OCCP increases the risk of thrombosis by 5-fold, and drospirenone, which is what our patient was using, specifically increases the risk by 6.3-fold [ 16 ].…”

“…[2][3][4][5] Pulmonary embolism often presents with chest discomfort, tightness, pain and difficulty in breathing. The clinical conditions frequently recorded in which pulmonary embolism occurs are deep vein thrombosis, following long journey in confined spaces such as aircrafts, post surgery, atrial fibrillation, congestive cardiac failure, sickle cell anemia 6 dehydration, during treatment with antipsychotics, 7 oral contraceptive usage, 8 and hypercoaguable states. In many instances it occurs without an underlying clinical condition and amongst these patients lupus anticoagulant and antiphospholipid antibodies are sometimes present.…”

“…COC-associated VTE is a very rare condition in Korean women, which has caused it to be overlooked by numerous physicians and healthcare professionals. A few case reports have described COC-induced VTE or arterial thrombosis in Korean women [10][11][12][13][14]. However, these describe a single case across different hospitals (Table 1), not as a single or multicenter case series.…”
