Pulmonary function derangements in isolated or predominant mitral stenosis — Preoperative evaluation with clinicohemodynamic correlation

Abstract: PFT derangements are seen in all grades of severity of MS and correlate well with the functional class, though no significant linear correlation with grades of severity of stenosis or pulmonary hypertension. Even the early or mild derangements in pulmonary function such as small airway obstruction in the less severe cases of normal or mild PH can be detected by simple and inexpensive methods when the conventional parameters are normal. The supplementary data from baseline arterial blood gas analysis is informa… Show more

“…The valvular lesions are unfortunately already highly evolved, mutilating, leaving little chance for conservative surgery, the patients are of ore and already at a stage of PAH very evolved. This PAH is a classical pathophysiologic consequence of left heart valvulopathies, and elevated pulmonary pressures result initially from increased left atrial pressure, pulmonary vasoconstriction, and ultimately obliterative changes in the pulmonary vascular bed; excessive thickening of the media and intimal fibrosis of pulmonary arterioles are typical changes following a long course of mitral stenosis [2][3][4].…”

“…Regarding BMI, there was no statistically significant difference between both groups. MIMVS has revealed lower morbidity and mortality in obese patients compared to conventional surgery [16][17] however in this study both groups were matched regarding BMI. 10 , report that MIMVS can patients at risk, and different encouraging reports have been published about patients with specific single comorbidities such as depressed left endocarditis, renal dysfunction, ventricular function 18 redo surgery, elderly and obesity 3,13 .…”

“…Однако работ, посвященных течению ХСН у пациентов с приобретенными пророками сердца, в частности ХрБС, немного [12]. а исследования, связанные с оценкой показателей функции внешнего дыхания у больных ХрБС, единичны [13] и проведены или на малом количестве пациентов (от 25 до 50), или связаны с больными, которым оценка параметров спирометрии проводилась до и после хирургического вмешательства [14,15].…”
