A 17-year-old man was occupationally exposed to pyromellitic acid dianhydride dust during the production of epoxy resin in a chemical factory. He was clinically diagnosed as having acute hemorrhagic alveolitis associated with anemia. The serologic analysis revealed a high concentration of IgG antibodies against pyromellitic acid dianhydride-treated human serum albumin (PMDA-HSA). Immunoblotting with PMDA-treated human serum as antigen and the patient's serum as the first antibody showed that additional PMDA-modified serum proteins other than HSA were recognized by the patient's IgG antibodies in the higher mol. mass range (> 67 kDa). No specific IgG could be detected against other anhydride conjugates (maleic acid, MA; phthalic acid, PA) with the exception of a reaction with the trimellitic acid anhydride-conjugated HSA (TMA-HSA). No specific IgE antibodies could be detected against any of the above mentioned antigens, but immunoblotting of the patient's serum indicated IgG4-type autoantibodies against in vitro PMDA-treated Ig molecules of normal serum proteins.

Diffuse alveolar hemorrhage (DAH) refers to a clinical syndrome resulting from injury to the alveolar capillaries, arterioles, and venules leading to red blood cell accumulation in the distal air spaces. It is defined by the clinical triad of hemoptysis, anemia, and progressive hypoxemia. Chest radiographs reveal non-specific patchy or diffuse, bilateral pulmonary infiltrates.The diagnosis requires confirmation of the alveolar hemorrhage by bronchoscopy in which serial bronchoalveolar lavage samples reveal persistently hemorrhagic fluid. A number of conditions are associated with DAH, and underlying disease determines the prognosis and the treatment regimen. While there is no uniformly accepted classification of DAH, it is generally categorized according to the underlying histology. EtiologyA number of conditions have been associated with the clinical syndrome of DAH (table 1). In one study of biopsyconfirmed DAH, Wegener's granulomatosis (WG) was the most frequent underlying condition, followed by Goodpasture's syndrome, idiopathic pulmonary hemosiderosis, and collagen vascular diseases. Overall, vasculitis (either WG or microscopic polyangiitis [MAP]) was the most frequent, representing 41% of cases [1].

The work-place and household exposure to different pollutants involves airways as well as parenchyma of the lung. We evaluated four patients without comorbidities, working in plastic industries at least for 11 years. Pulmonary function testing of the patients revealed mixed airway disease. Computed tomography revealed bronchiectasis, centrilobular nodule, septal thickening and emphysema unrevealing the toxic potential of plastic fumes. The use of personal protective equipment with good work hygiene practises would certainly help in the prevention of these occupation-related morbidities.
