Pulmonary hypertension in patients with Martorell hypertensive leg ulcer: a case control study

Abstract: BackgroundMartorell hypertensive ischemic leg ulcer (Martorell ulcer) is characterized by distinct alterations in the arteriolar wall of subcutaneous vessels, leading to progressive narrowing of the vascular lumen and increase of vascular resistance. These changes are similar to the alterations observed in pulmonary arterioles in patients with chronic pulmonary hypertension (PH). This study was aimed to assess an association between the two disorders.MethodsIn this case–control study, 14 patients with Martorel… Show more

“…At the same time, the vascular compensation mechanisms are also impaired 84 , and capillary density is reduced. A comparison with other patient populations shows high vascular comorbidity, including an increased risk of pulmonaly hypertension 79,85 . This is a logical consequence of pathophysiology, since arterial hypertension affects all organs.…”

“…Gleichzeitig sind dadurch auch die vaskulären Kompensationsmechanismen beeinträchtigt 84 und die Kapillardichte ist reduziert. Der Vergleich mit anderen Patientengruppen zeigt eine hohe vaskuläre Komorbidität, einschließlich einem erhöhten Risiko für pulmonale Hypertension 79,85 . Dies ist letztlich eine logische Folge der Pathophysiologie, da alle Organe der arteriellen Hypertonie ausgesetzt sind.…”

“…The ischaemic wound border is discerned by a violaceous hue and an undermined appearance. Individuals afflicted by Martorell HYTILU endure intense pain, disrupted sleep, and a pervasive sense of exhaustion (1)(2)(3). Histological examination reveals subcutaneous arterioles exhibiting thickened walls, often at the expense of a narrow lumen.…”
