Punktuelle Excimerlaserablation des Trabekelwerks

Abstract: With the microsurgical method of pinpoint ablation of the trabecular meshwork by excimer laser, intraocular pressure was reduced in 30 of 35 eyes over a median follow-up period of 7 months. These results encourage us to continue the development of this procedure, perhaps with a microendoscope. The minimal trauma to the eye of this procedure leaves all other options of surgery open.

“…Neuhann et al [17] and Vogel and Lauritzen [16] reported results at 6 and 7 months, respectively, similar to ours, whereas results reported by other authors [18] were only a little better in a study group also involving eyes affected by cataract and submitted to a combined procedure consisting of ELT and phacoemulsification with intraocular lens implantation.…”

“…It was developed to improve the aqueous outflow by creating holes through the anterior meshwork of the inner wall of Schlemm's canal with minimal thermal or necrotic effects on the trabecular meshwork. [31][32][33] Babighian et al 33 compared 2-year IOP outcomes of ELT and 1801 SLT in an RCT for patients with POAG refractory to medical therapy. Eyes treated with ELT resulted in a mean IOP reduction from 25.0±1.9 mm Hg at baseline to 17.6±2.2 mm Hg at 24 months.…”

“…Finally, the authors do not propose a hypothesis to explain the progressive decrease in intraocular pressure over time as opposed to other ELT studies in which the decrease in intraocular pressure occurs in the immediate postoperative period and is sustained [4,5] . Do the authors offer any hypothesis as to why their patients have a slower rate of decline than others?…”
