Pure Testicular Choriocarcinoma with Dermatological, Brain, and Gastrointestinal Metastases

Abstract: Testicular choriocarcinoma is a non-seminomatous germ cell tumor (NSGCT) and is the rarest of all testicular cancers. Nearly all choriocarcinomas can be classified as either pure choriocarcinoma or as a component of a mixed germ cell tumor. Pure testicular choriocarcinoma is extremely aggressive and metastasizes early and extensively. We present a case of testicular cancer that metastasized to the skin, gastrointestinal tract, and brain, and discuss the case in light of the available literature.

“…This is caused by early haematogenous spread, with distant lesions often affecting the GI tract, lung and brain. These tumours are associated with local vascular invasion, and thus commonly present with haemorrhagic symptoms such as GI bleed, haemoptysis or cranial/CNS haemorrhage 1–3. As was the case for our patient, the urgency of these haemorrhagic complications and associated morbidity/mortality, may be more pressing than the oncological disease treatment 1 2…”

“… 10 Sites of metastasis such as skin have been rarely reported and are associated with poor prognosis. 11 This applies to our patient as his condition deteriorated shortly after the appearance of the cutaneous lesion.…”
