“…By reviewing relevant literature, the potential mechanism of secondary AP of KP-PLA is proposed here: blood infection: Klebsiella pneumoniae was cultured in the drainage fluid and blood of some KP-PLA patients with secondary extrahepatic infection, suggesting that Klebsiella pneumoniae could cause extrahepatic infection through blood transfer. [3,9] A retrospective study suggested that KP-PLA patients with sepsis were more prone to extrahepatic infection, suggesting that hematogenous infection was a viable route. [10] Biliary tract infection: According to the anatomical structure of liver and pancreas, the intrahepatic and intrahepatic bile ducts eventually converge to the common bile duct and accompany the pancreatic duct to eventually open at the duodenal papilla; part of the common bile duct may converge with the pancreatic duct and open at the duodenal papilla; bacterial infection in the liver may enter the pancreatic tissue through the biliary tract system and lead to AP.…”

“…This trend is especially relevant from the point of view of ophthalmologists because liver abscess due to Klebsiella is associated with the metastatic spread (in 12% of cases, compared to <1% in non-Klebsiella etiologies) [ 1 , 19 ]. The most common manifestations are meningitis and endophthalmitis, however bacteremia, pneumonia, splenic abscess, and necrotizing fasciitis may also occur [ 24 ]. The actual incidence of EE in Klebsiella liver abscess ranges from 0.9 to 7.9% [ 25 ].…”

“…Endogenous endophthalmitis (EE) is a rare and serious complication of PLA and often appears with eye symptoms before PLA 8,19,20 . Despite continued negative blood cultures, the patient's presentation may be consistent with invasive liver abscess syndrome, with associated EE qualifying as a systemic manifestation of disease 21 . A meta‐analysis estimates the actual incidence of EE among PLA patients, where EE is reported in approximately one out of every 22 patients with Klebsiella pneumoniae pyogenic liver abscess.…”

K1 capsular serotype infection was an independent risk factor.…”
