Abstract: Nutritional variant streptococcus (NVS) or pyridoxal-dependent streptococcus is a rare but significant cause of infective endocarditis (IE), which presents as a diagnostic dilemma due to difficulty in organism isolation, and high rates of treatment failure, recurrence, and mortality.
