Qualitative Data Collection and Analysis Methods: The INSTINCT Trial

Abstract: Patient care practices often lag behind current scientific evidence and professional guidelines. The failure of such knowledge translation (KT) efforts may reflect inadequate assessment and management of specific barriers confronting both physicians and patients at the point of treatment level. Effective KT in this setting may benefit from the use of qualitative methods to identify and overcome these barriers. Qualitative methodology allows in-depth exploration of the barriers involved in adopting practice cha… Show more

“…Only two papers explicitly stated that they used theory to explore barriers and enablers; using a conceptual model of the innovation in diffusion theory18 to analyse the results of focus groups and to determine the barriers to implementation of a paediatric asthma management protocol19 and the Cabana taxonomy20 to assess barriers to behaviour change for the appropriate use of tissue plasminogen activator (tPA) in ischemic stroke 21…”

“…It is acknowledged that this paper is limited in its ability to track papers as only 3 years were explored. The ability to link papers is dependent on the years that the papers were published, for example, the protocol for one of the larger RCTs (INSTINCT)21 was published in 2007, but the barrier analysis and trial results were excluded as they were published in 201138 and 2013 39. There were, however, very few papers that included references in the background or discussion of papers evaluating a preliminary gap or barrier analysis; so, it can be assumed that these stages were not undertaken or not published.…”

“…Its goal was to determine whether a multifaceted intervention using opinion leaders, targeted messaging on stroke care, performance feedback, and a hotline for physician support increases rates of tPA use among emergency departments and physicians in community hospitals. The intervention design was guided by review of existing literature and a qualitative study of barriers 34…”

“…For these inputs, we were forced to make assumptions or utilize estimates from manufacturing and clinical experts working with telestroke. Third, telestroke is just one of many methods for increasing tPA usage for acute stroke patients, with other methods including targeted physician education 35 and telephone-only consultations. 36 In randomized trials of telephone-only vs telestroke consultations, the telephone-only consultations show poor sensitivity for ruling in tPA-eligible patients.…”
