Quality assessment of an interferon-gamma release assay for tuberculosis infection in a resource-limited setting

Abstract: BackgroundWhen a test for diagnosis of infectious diseases is introduced in a resource-limited setting, monitoring quality is a major concern. An optimized design of experiment and statistical models are required for this assessment.MethodsInterferon-gamma release assay to detect tuberculosis (TB) infection from whole blood was tested in Hanoi, Viet Nam. Balanced incomplete block design (BIBD) was planned and fixed-effect models with heterogeneous error variance were used for analysis. In the first trial, the … Show more

“…We re-performed ELISA for all preserved plasma samples with indeterminate results and obtained completely the same results. In addition, we have paid careful attention to maintain the high quality of this test [16] . All of the indeterminate cases showed low response to both TB antigen and mitogen and the pattern of TST measurements in IGRA indeterminate cases was similar to that of IGRA negative cases.…”

“…Based on the algorithm of the software, the result was considered to be indeterminate in one of the following two conditions: the nil value itself was higher than 8.0 IU/ml, or mitogen minus nil value was less than 0.5 IU/ml in addition to TB antigen minus nil was less than 0.35 IU/ml. The testing procedure was carefully monitored [16] and quality control of the test was done in each run, following the manufacturer's instruction.…”

“…Interferon-gamma concentrations were calculated by subtracting negative control values from TB antigen-stimulated values, and the cut-off value was 0.35 IU/ml. The testing procedure was carefully monitored [ 15 ] and quality control was included in each run according to the manufacturer’s instructions.…”

“…The algorithm and software (QuantiFERON-TB Gold Analysis Software, version 2.50, Cellestis) provided by the manufacturer were strictly followed for interpretation of the results [11] . The testing procedure was carefully monitored [12] and quality control of the test was done in each run, following the manufacturer's instructions. For analysis of IFN-γ values higher than 10.00 IU/ml, the truncated value (10.00 IU/ml) was used as indicated in the current software.…”
