Quality of Care Associated with Common Chronic Diseases in a 9-State Medicaid Population Utilizing Claims Data: An Evaluation of Medication and Health Care Use and Costs

Abstract: The objective of this cross-sectional, retrospective study was to utilize claims data to establish a quality-of-care benchmark in a large multistate Medicaid population overall and by race. Quality of care and medication adherence (persistence and compliance) per national treatment guidelines, and health care costs/utilization were assessed across common chronic conditions in a large, 9-state Medicaid population. Overall, quality of care was suboptimal across conditions. Over 15% of asthma patients had 1 asthm… Show more

“…Even among those patients who did refill the LTC-Rx, only 16% were consistent in adhering to therapy for 6 months (i.e. medication possession ratio >0.8) [20]. In the four-state sample of the National Asthma Survey, over half (52.8%) of patients with persistent asthma did not report receiving an inhaled corticosteroid, even though they should have been receiving a long-term controller therapy.…”

“…HCC categories were grouped into 5 major chronic disease groups for risk adjustment: Cerebral (HCC 097–099), Heart (HCC 083–084), Vascular (HCC 104–105), Cancer (HCC 007–010), Diabetes (HCC 015–020) and Arrhythmias (HCC 092–093). These conditions are common among veterans and are associated with higher costs of care [1821]. A patient might be found in multiple conditions, if more than one was documented.…”

“…Across 9 states, only 27% of adult asthma patients on Medicaid refilled a prescribed longterm controller medication prescription (LTC-Rx) at least twice. 18 Among patients who did refill the LTC-Rx, only 16% adhered to therapy consistently for 6 months. Higher adherence to long-term controller therapy is associated with lower ED visit rates, 19,20 but ICS-Rx adherence under ''real world'' conditions among Medicaid-enrolled children with asthma is only 20%.…”
