Objective: This study aims to examine the overall and domain-specific quality of life in patients with stroke and to identify variables predicting quality of life after stroke. Material and Methods: A total of 104 patients with sufficient cognitive functions having hemiplegia because of cerebrovascular accident and 108 controls were included in this study. Demographic and clinical features were recorded. The Barthel Index (BI) and Functional Ambulation Category scale were employed to assess the functional state of patients. Quality of life was evaluated by the Short Form 36 (SF-36) health survey, whereas the emotional state of the patients was evaluated by the Hospital Anxiety and Depression Scale (HADS). Functional status, motor impairment, and emotional state were determined as independent variables, and multiple regression analysis was used to predict quality of life. Results: Patients had significantly lower scores in the quality of life subscores and total scores in comparison with the controls (p<0.001). The mean anxiety and depression rates in the patients were significantly higher than those in controls (p<0.001). The sum of subscores and physical composite score of SF-36 were significantly lower in female and illiterate patients than in male and educated patients. Multiple regression analyses indicated that HADS depression was associated with mental health (p<0.001), whereas BI was independently associated with the physical health total score (p<0.05). The power of the statistical analysis for study population was 100% according to the given effect size (α=0.01). Conclusion: Present results demonstrated that patients with stroke have a significantly poor quality of life than the general population. The reduced quality of life after stroke appears to be related the emotional state, physical disability, and demographic properties such as gender and education. Therefore, prevention of disability and early diagnosis and treatment of depression are vital in improving the quality of life of patients with stroke.

Purpose: This study aims to (1) assess differences in participation restrictions between stroke survivors aged under and over 70 years and (2) identify predictors associated with favorable and unfavorable longterm participation in both age groups. Methods: Prospective cohort study in which 326 patients were assessed at stroke onset, two months and one year after stroke. The Utrecht Scale for Evaluation of Rehabilitation-Participation (USER-Participation) was used to measure participation restrictions one year after stroke. Bivariate and multivariate logistic regression analyses were performed including demographic factors, stroke-related factors, emotional functioning and comorbidity as possible predictors. Results: Stroke survivors aged over 70 years perceived more participation restrictions in comparison to stroke survivors aged under 70 years one year after stroke. Independently significant predictors for unfavorable participation outcomes were advancing age, more severe stroke and anxiety symptoms in patients aged over 70 years, and female gender, more severe stroke, impaired cognition and depression symptoms in patients aged under 70 years. Lower age was the only independent predictor associated with favorable participation after one year in stroke survivors aged over 70 years. Conclusions: This study emphasizes the need to pay more attention to participation restrictions in elderly stroke survivors. ä IMPLICATIONS FOR REHABILITATIONMore attention in the rehabilitation process should be paid to restrictions in participation of stroke survivors aged older than 70 years, taking into account the different participation needs and predictors of older stroke survivors. Early screening on the presence of anxiety symptoms could potentially prevent long-term restrictions in participation in stroke survivors aged over 70-year old. Stroke survivors experience considerable restrictions in physical activity and mobility after one year, highlighting the need for the development of community-based exercise programs for stroke survivors.ARTICLE HISTORY

ABStRAct:In clinical practice, the physical therapist often evaluates and programs the patient's treatment without taking into account the temporal variation of functions and behaviors. The aim of this study was to analyze the influence of the chronotype (morning type -evening type), sleep-wake state (sleep quality and excessive sleepiness) and lifestyle regularity in determining the preferred time of day for physical and mental activities following stroke. Participants responded to the MorningnessEveningness Questionnaire (MEQ), the Pittsburgh Sleep Quality Index (PSQI), the Epworth Sleepiness Scale (ESS) and the Social Rhythm Metric (SRM). The subjects were asked at what time of day they preferred to perform physical (exercises) and mental (tasks of reasoning) activities, considering only their well-being. A total of 42 patients (61±9 years) in the chronic stage after stroke (18±21 months) and 12 healthy individuals took part in the study. The data were analyzed using the χ 2 test and multiple regression. Among the patients evaluated, 64% had poor sleep quality, 43% excessive sleepiness and 57% an irregular daily routine. Regression analysis showed that chronotype was the only factor studied that influenced the preferred time of day for the activities. The results indicate the need for the physical therapist to analyze how chronotype can affect patient performance before establishing time of day of the physical therapy sessions.
