Quantification of Prostate Cancer Metabolism Using 3D Multiecho bSSFP and Hyperpolarized [1‐13C] Pyruvate: Metabolism Differs Between Tumors of the Same Gleason Grade

Abstract: Background: Three-dimensional (3D) multiecho balanced steady-state free precession (ME-bSSFP) has previously been demonstrated in preclinical hyperpolarized (HP) 13 C-MRI in vivo experiments, and it may be suitable for clinical metabolic imaging of prostate cancer (PCa). Purpose: To validate a signal simulation framework for the use of sequence parameter optimization. To demonstrate the feasibility of ME-bSSFP for HP 13 C-MRI in patients. To evaluate the metabolism in PCa measured by ME-bSSFP. Study Type: Retr… Show more

“…Several approaches have been demonstrated to boost P to the order of unity, i.e., several orders of magnitude above the equilibrium level, to improve NMR detection sensitivity. , Such hyperpolarized (HP) states are created transiently on small exogenous molecules used as contrast agents that can be injected in vivo to probe metabolic transformations. Hyperpolarized 13 C-labeled carboxylates are favored because they can retain an HP state for minutes in vivo. , HP [1- 13 C]pyruvate is the most developed HP contrast agent, and it is now under evaluation in over 30 clinical trials according to clinicaltrials.gov.…”

“…[1] This carries valuable diagnostic information, since abnormal pyruvate metabolism is associated with various pathologies (e.g., tumour cells exhibit accelerated pyruvate-to-lactate conversion; "Warburg effect"). [2] Pyruvate has emerged as the leading HP MRI agent because of fast cellular uptake and its central position in cellular metabolism at the cross-section of glycolysis, the citric acid cycle, alanine transamination, and lactate fermentation. [3] HP pyruvate is under evaluation in over 30 clinical trials as off 2022.…”
