Quinolone Arthropathy in Animals Versus Children

Abstract: The use of quinolones in children and accumulation of data on the pharmacodynamics of these drugs have been limited and delayed by concern regarding their chondrotoxicity. A comprehensive review of the findings in animals compared with the cumulative published findings in children and adolescents (>7,000 to date) allows the conclusion that such concern is not justified. Prospective controlled studies in children are justifiable in view of a continuing lack of correlation between findings in juvenile animals an… Show more

“…However, ultrasound examination and nuclear magnetic resonance imaging scans showed no evidence of cartilage toxicity in ciprofloxacin-treated CF patients [133,137]. A comprehensive review of the findings in animals compared with the cumulative published findings in children and adolescents allowed the conclusion that such concerns were not justified [138]. On this topic a consensus meeting has been held [139,140].…”

“…The risk of arthropathy, as described for juvenile animals, is currently regarded to be low or even negligable [113,114]. Of 12 children with ulceroglandular tularaemia treated as outpatients with oral ciprofloxacin, all responded satisfactorily [106].…”

“…Traditionally, the use of quinolones in children has been contraindicated for this reason. However, children with cystic fibrosis given ciprofloxacin, norfloxacin, and nalidixic acid have had few, and reversible, joint symptoms [53]. Therefore, in some cases the benefits may outweigh the risks of quinolone therapy in children.…”
