Radiofrequency ablation with four electrodes as a building block for matrix radiofrequency ablation: Ex vivo liver experiments and finite element method modelling. Influence of electric and activation mode on coagulation size and geometry

Abstract: Radiofrequency ablation (RFA) is increasingly being used to treat unresectable liver tumors. Complete ablation of the tumor and a safety margin is necessary to prevent local recurrence. With current electrodes, size and shape of the ablation zone are highly variable leading to unsatisfactory local recurrence rates, especially for tumors > 3 cm. In order to improve Table 6: outcome of the ex vivo bovine liver experiments: comparison according to the number of electrodes activated at the same time.

“…Although the liver is commonly used for the assessment of the distribution and efficacy of ablative therapies [48][49][50][51][52][53][54][55][56], we plan to expand this work in the future to include other healthy tissues as well as cancerous lesions. Future studies should elucidate the effect of tissue biophysical differences (i.e., vascularity, stiffness, density).…”

“…While the swine liver differs from the human liver in external morphology, it has a similar size, segmental anatomy, vascularity, and biliary tree structure [47]. The efficacy of ablative therapies is commonly examined and in healthy liver tissue [48][49][50][51][52][53][54][55][56] specifically, traditional ethanol ablation [32,48,57] (PEI), radiofrequency ablation [54,58] (RFA), microwave ablation [55,[59][60][61] (MWA), drug-eluting bead embolization [62], and high-intensity focused ultrasound [63] (HIFU) are used. These relatively homogeneous tissues provide a consistent environment for such studies.…”

“…The clinician manipulates the internal structures through small incisions to make the treatment region accessible and to ensure that the treatment is performed on the right location. LLA has four different types: Radiofrequency Ablation (RFA) [32][33][34]48 , Microwave Ablation (MWA) [48][49][50][51] , Irreversible Electroporation (IRE) 52 , Cryoablation (CA) [53][54][55] and Ethanol Injection (EI) [56][57][58] Modeling strategies. To establish a generic process model of MILT, the modeling strategies proposed in our previous work 10 were applied as described below.…”

“…While on the other side, the authors simulated FEM to ablate breast cancer by using RFA and succeeded in abating 80% of the tumor [18]. In [19], the authors designed FEM to ablate liver tumors by using cool-tip RF electrodes.…”
