Radiographic findings related to interlocking nailing: windshield-wiper effect, and locking screw failure

Abstract: SummaryThe purpose of this paper is to describe two uncommon radiographic findings related to the use of interlocking nails (ILNs) fixation. The first one is a radiolucency located around the distal tip of the nail observed in three clinical cases. “…One of the crucial issues with this patient was nonunion at the fracture site with bony lytic expansile lesion at distal tip of nail due to instability at the fracture site with distal locking screw missing the nail making it an unstable construct traditionally called Windshield wiper effect which has been seen and reported in pediatric patients and animals too [ 11 , 12 ]. With instability at fracture site which was resulting from unstable fixation as the distal locking screw of nail was outside the nail resulting in toggling of nail inside medullary canal.…”
