Radiography of Cardiac Conduction Devices: A Pictorial Review of Pacemakers and Implantable Cardioverter Defibrillators

Abstract: Cardiac conduction devices (CCDs) depend on correct anatomic positioning to function properly. Chest radiography is the preferred imaging modality to evaluate CCD's anatomic location, lead wire integrity, and help in identifying several complications. In this pictorial review, our goal is to familiarize radiologists with CCD implantation techniques, appropriate positioning of the device, common causes of malfunction, methods to improve report accuracy, and assure maximal therapeutic benefit.

“…Additionally, differentiating between ICD and pacemakers on chest x-ray is imperative to ensure adequate anesthesiology pre-operative prep. To distinguish a pacemaker vs. ICD on chest x-ray, Pacemakers have small leads (Figure 6a), where ICD's have thick coiled segments at the end of their leads (Figure 6b) [37].…”

“…Every ICD lead also has built-in pacemaker functionality, even if pacing was not indicated. Diff erent companies and models have distinct shapes on an x-ray (2,3). If the device cannot be identifi ed radiologically, diff erent company devices can be applied to identify the device make and model, as a last resort.…”

“…Chest radiographs are the initial modality to evaluate the device location and its integrity after implantation and for diagnosis of complications and malfunction. [ 25 , 26 , 27 ]. Antibacterial properties of gold were also desired as a potential lowering of implant failure in the early stages after operation time due to the reluctance of bacteria to settle on the surface [ 28 ].…”
