Radiological and functional outcome in extra-articular fractures of lower end radius treated conservatively with respect to its position of immobilization

Abstract: Background:Extra-articular fractures of lower end radius are conventionally immobilized in palmar flexion and ulnar deviation. In view of poor functional results, the conventional method of immobilization is giving way to dorsiflexed-immobilized method. The aim of our study is to evaluate and compare the radiological and functional outcome in extra-articular fractures of lower end radius treated conservatively with respect to its position of immobilization.Materials and Methods:Sixty-four patients, all above 2… Show more

“…We found that individual movements of supination, pronation, ulnar and radial deviation as well as total range of movements are satisfactory when the wrist is immobilized in dorsiflexion, the fact that has equally being concluded by earlier studies. 6,7 Further, recovery of grip strength and subjective assessment of pain, disability and limitation of the movements were also encouraging.…”

“…7 Also the wrist in extension is the optimal position for hand function and rehabilitation of the fingers. 6 In this study we compared the functional and radiological results of extra-articular Colles type of distal radius fractures treated conservatively with wrist immobilized in dorsiflexion. We found that individual movements of supination, pronation, ulnar and radial deviation as well as total range of movements are satisfactory when the wrist is immobilized in dorsiflexion, the fact that has equally being concluded by earlier studies.…”

“…The time of casting and the length of the cast are two causes that impact the incidence of complications. Prolonged immobilization may lead to joint contracture, muscle atrophy and weakness, disuse osteopenia and possibly functional hardship that affect the patient's function (26). Edmonds et al compared the duration of immobilization that is required for good align-ment of the fracture line in the short cast with the long cast (27).…”

“…26,27 The acute palmar flexed position of the wrist during the postoperative immobilization period was blamed as the main reason for stiffness. 28 Some surgeons advocate treatment by manipulation and plaster immobilization. 29 Many recommend operative intervention as the only methods to obtain anatomical reduction, and some have proposed that the best functional result will only be achieved by obtaining as near an anatomical radiographic result as possible.…”
