We examined homes of hookah-only smokers and nonsmokers for levels of indoor air nicotine (a marker of secondhand smoke) and indoor surface nicotine (a marker of thirdhand smoke), child uptake of nicotine, the carcinogen 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanone (NNK), and the toxicant acrolein by analyzing their corresponding metabolites cotinine, 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanol (NNAL) and NNAL-glucuronides (total NNAL) and 3-hydroxypropylmercapturic acid.

Background: Benzene is a human hematotoxicant and a leukemogen that causes lymphohematopoietic cancers, especially acute myelogenous leukemia. We investigated uptake of benzene in hookah smokers and non-smokers attending hookah social events in naturalistic settings where hookah tobacco was smoked exclusively.Methods: We quantified S-phenylmercapturic acid (SPMA), a metabolite of benzene, in the urine of 105 hookah smokers and 103 non-smokers. Participants provided spot urine samples the morning of and the morning after attending an indoor hookah-only smoking social event at a hookah lounge or in a private home.Results: Urinary SPMA levels in hookah smokers increased significantly following a hookah social event (P < 0.001). This increase was 4.2 times higher after hookah lounge events (P < 0.001) and 1.9 times higher after home events (P ¼ 0.003). In non-smokers, urinary SPMA levels increased 2.6 times after hookah lounge events (P ¼ 0.055); however, similar urinary SPMA levels were detected before and after home events, possibly indicating chronic exposure to benzene (P ¼ 0.933).Conclusions: Our data provide the first evidence for uptake of benzene in hookah smokers and non-smokers exposed to hookah tobacco secondhand smoke at social events in private homes compared with their counterparts in hookah lounges. Hookah tobacco smoke is a source of benzene exposure, a risk factor for leukemia.Impact: Because there is no safe level of exposure to benzene, our results call for interventions to reduce or prevent hookah tobacco use, regulatory actions to limit hookah-related exposure to toxicants including benzene, initiate labeling of hookah-related products, and include hookah smoking in clean indoor air legislation. SOUHRNPráce je zaměřena na problematiku kouření u lékařů a zdravotnických pracovníků a na názory studentů medicíny na tento problém v souvislosti s jejich budoucí intervencí k zanechání kouření u svých pacientů.Průzkum na Lékařské fakultě Ostravské univerzity v Ostravě byl proveden pomocí standardizovaného dotazníku mezinárod-ní studie GHPSS u 255 studentů všeobecného lékařství 1.-4. ročníku. U studentů bylo zjišťováno kuřáctví a to nejen cigaret, ale i doutníku, vodní dýmky nebo šňupání tabáku. Byly zjišťovány názory na současnou protikuřáckou politiku a na specifickou úlo-hu zdravotníků v podpoře odvykání kouření.Z výsledku průzkumu vyplývá, že kouří 25 % všech studentů. Byl zjištěn statisticky významný rozdíl ve výskytu kuřáctví mezi jednotlivými ročníky. Studenti nižších dvou ročníků byli častějšími uživateli tabákových výrobků než studenti vyšších ročníků. Vět-šina studentů si je vědoma, že je nutné zaznamenávat u pacientů anamnézu kouření jako součást osobní anamnézy.Klíčová slova: kouření mediků, prevence kouření SUMMARYThe paper deals with smoking among doctors and medical personnel and the opinions of medical students regarding this problem in terms of their future intervention as doctors in helping their patients to stop smoking.A questionnaire survey among 255 medical students in the 1st to 4th years of study was performed at the Faculty of Medicine, University of Ostrava using the standardised to GHPSS international study questionnaire. Consumption of cigarettes water pipes, cigars and snuff was evaluated, as well as opinions on current anti smoking policy and the specific role of doctors in prevention of smoking.Results showed that 25% of students are smokers with statistically significant differences among individual years of study. Students in the first two yars of study used tobacco product more often than the highest years. The majority of students are aware of the fact that smoking status should be included in patient anamnesis.Key words: smoking among medical students, prevention of smoking Ostravská univerzita v Ostravě, Lékařská fakulta, Ústav epidemiologie a ochrany veřejného zdraví, OstravaÚvod Užívání tabáku usmrtí více než 5 milionů lidí roč-ně. Kouření patří mezi pět nejzávažnějších rizikových faktorů úmrtnosti. Jedná se o celospolečenský zdravotnický problém, který způsobuje vážná kardiovaskulár-ní, onkologická a další onemocnění. Pokud bude současný stav pokračovat, do roku 2030 zemře na násled-ky kouření více než 8 milionů lidí ročně. Kromě zdravotnických důsledků nelze opomenout ani ekonomické problémy, které nastávají v důsledku kouření. Užívá-ní tabákových výrobků je neoddělitelně spjato s chudobou. V zemích s nízkými a středními příjmy mohou vý-daje na tabákové výrobky představovat až 10 % z celkového příjmu rodiny (1).Kouření u lékařů a zdravotnických pracovníků hraje významnou roli v prevenci užívání tabáku a klíčo-vé postavení v rozvoji politiky veřejného zdravotnictví.
