“…Interim results of the per protocol population of our trial have been reported previously (Bamberg et al, 1999) lending further evidence to the role of para-aortic radiotherapy as the new standard of radiotherapy in stage I seminoma. We now present an intention-to-treat analysis of the entire study population after a median time to follow-up of 61 months.…”

“…Gunnlaugsson et al 16 reported a strong correlation between the amount of small bowel irradiated to doses larger than 15 Gy and gastrointestinal side effects. Bamberg et al 17 found acute gastrointestinal side effects more prevalent in irradiated Stage II compared to irradiated Stage I patients that could be attributed to the additional boost or to larger total doses used in the past. It is clear that dose reduction should translate into lower rates of acute and chronic side-effects.…”

“…Several clinical trials have aimed at limitation of dose and extent of adjuvant radiotherapy in stage I disease in order to further reduce the already low incidence of late treatment-related morbidity while leaving the excellent cure rate unchanged [12Á/14]. Special interest has been focused on minimizing the scatter dose to the remaining testis leading to treatment volumes confined to the para-aortic lymph nodes [5,6]. In contrast with the lower field borders the upper border of the para-aortic fields at the T10/T11 level has not been questioned by radiation oncologists so far although the surgical approach Abbreviations: RTI0/rete testis invasion; V 0/vascular invasion; Ln0/lymph node(s); PEI0/cisplatin, etoposide, and ifosfamide; PEB0/ cisplatin, etoposide, and bleomycin; Carbo0/carboplatin; NED0/no evidence of disease.…”
