Randomized clinical trial of suction versus standard clearance of the diathermy plume

Abstract: Suction clearance of the diathermy plume resulted in a significant reduction in the amount of smoke reaching the level of the operator's mask. Although the risk of diathermy smoke inhalation is currently unknown, use of such a system appears advisable.

“…Despite numerous studies, the composition of smoke produced by electrosurgery remains unclear and a number of studies report the difficulties in obtaining reproducible results from real electrosurgical smoke investigations [2,5]. Some researchers report the presence of BTEX compounds [1,5] whereas others could not identify benzene or other BTEX components in vivo [8] or in simulation studies [9].…”

“…Numerous studies have attempted to determine the risks that these aerosols pose both to patients and surgeons [1]. As a result, protective measures such as the use of masks and surgical smoke evacuation systems have been recommended [2]. Unprotected exposure to electrosurgical by-products is still a common practice in many operating theatres.…”

“…As surgical masks do not provide protection [10,11], the exposure to surgical aerosols should be aimed at being minimised at all times. This can probably easily be achieved by a simple offtake [12]. Taking the individual and constantly changing labour situations into account, further measurements during daily routine are necessary.…”

“…3,6,18,19 It has been recognised that the potential for future litigation for occupational exposure must be factored into any economical considerations for using smoke extractors. 3 Although there will be reservations about adopting new systems, many have been implemented in the past and proved successful.…”
