Aim (a) To investigate the influence of the keratinized mucosa (KM) on peri‐implant health or disease and (b) to identify a threshold value for the width of KM for peri‐implant health. Materials and Methods The total dataset was subsampled, that is one implant was randomly chosen per patient. In 87 patients, data were extracted at baseline (prosthesis insertion) and 5 years including the width of mid‐buccal KM, bleeding on probing, probing depth, plaque index and marginal bone level (MB). Spearman correlations with Holm adjustment for multiple testing were used for potential associations. Results Depending on the definition of peri‐implant diseases, the prevalence of peri‐implantitis ranged from 9.2% (bleeding on probing threshold: <50% or ≥50%) to 24.1% (threshold: absence or the presence). The prevalence of peri‐implant mucositis was similar, irrespective of the definition (54%–55.2%). The width of KM and parameters for peri‐implant diseases demonstrated negligible (Spearman correlation coefficients: −0.2 < ρ < 0.2). No threshold value was detected for the width of mid‐buccal KM in relation to peri‐implant health. Conclusion The width of KM around dental implants correlated to a negligible extent with parameters for peri‐implant diseases. No threshold value for the width of KM to maintain peri‐implant health could be identified.

Aim:To compare the implant survival rate between short dental implants and standard length implants placed in combination with bone grafting at 5 years of loading. Methods: This multicentre study enrolled 101 patients (137 implants) with a posterior maxillary bone height of 5-7 mm. Patients randomly received either short implants (6 mm; GS) or long implants (11-15 mm) with sinus grafting (GG). Six months later, implants were loaded with single crowns and patients re-examined at 1, 3 and 5 years of loading. Outcomes included: implant survival, marginal bone levels (MBLs), biological and technical parameters and patient-reported outcome measures (OHIP-49 = Oral Health Impact Profile). Statistical analysis was performed using a nonparametric approach.Results: At 5 years, 90 patients (124 implants; GS: 60; GG: 64) were re-examined (drop-out rate 10%). Patient-level implant survival rates were 98.5% (GS; 1 implant failure) and 100% (GG; p = 0.49). Mean MBLs were 0.54 mm ± 0.87 (GS) and 0.46 mm ± 1.00 (GG; p = 0.34). Biological and technical parameters were not significantly different (p > 0.05). Median overall OHIP-49 scores improved significantly up to 5 years in both groups (GS: p = 0.03; GG: p = 0.00; intergroup comparison p = 0.11). Conclusions:Both treatment modalities were suitable for implant therapy in the atrophied posterior maxilla revealing no differences in terms of survival rates, marginal bone levels (changes), patient-reported outcomes and technical/biological complications.

There are two main groups of screw‐type dental implant designs: one‐piece and two‐piece implants. Although success rates of both of these types of implants are high, none of them avoid complications, of which the most common are peri‐implant mucositis and peri‐implantitis. Current clinical diagnostic parameters are relatively noninvasive and cost‐efficient; however, they are often not sensitive enough and fail to determine the activity of inflammation. The purpose of this study is to determine pH of peri‐implant crevicular fluid (PICF) around one‐piece and two‐piece implants and pH of gingival crevicular fluid (GCF) around healthy teeth and to find out if our suggested method could function accurately for determination of pH of PICF and GCF. Thermo Fisher Scientific™ Orion™ 9863BN glass microelectrode was used to determine pH of PICF around 29 one‐piece (ROOTT, TRATE AG) and 29 two‐piece implants (multiple manufacturers) and pH of GCF around 29 healthy teeth. pH of PICF around two‐piece implants was more acidic (P < 0.001). Average pH around one‐piece implants was 6.46 and around two‐piece implants was 6.15. Mean pH of GCF was 6.64. pH of PICF in women around two‐piece implants was more alkaline (P < 0.05); no difference was found in control and one‐piece implant groups. There was no statistically significant correlation found between age and pH of PICF and GFC. Design of dental implants has an impact on pH of PICF. Glass microelectrode is an appropriate tool for accurate determination of pH in PICF.
