Randomized trial of MST and ARC in a two-level evidence-based treatment implementation strategy.

Abstract: A two-level strategy for implementing evidence-based mental health treatment was assessed in a 2 × 2 randomized trial with 615 delinquent youth in 14 rural Appalachian counties. The implementation strategy included (1) the introduction of a Multisystemic Therapy (MST) program for delinquent youth in each county, and (2) the ARC (for Availability, Responsiveness and Continuity) organizational intervention for implementing effective community-based mental health services in randomly assigned counties. “…Five randomized controlled trials (RCTs) of ARC have been conducted in the Midwest, Northeast, and Southeast regions of the United States. These RCTs show that ARC improves organizational social context, increases job satisfaction and commitment, supports EBT implementation, reduces staff turnover, and improves service outcomes (33,38,39,42,43).…”

“…Similarly, MDFT has been housed in juvenile detention centers to build a continuum-of-care bridge between detention and discharge to community supervision, where it significantly increased treatment linkage and retention (Liddle, Dakof, Henderson, & Rowe, 2011). On an even larger scale, MST has been combined with intensive implementation strategies to improve behavioral health across multiple rural communities (Glisson et al, 2007), FFT has been adopted in statewide initiatives to improve child welfare and juvenile justice outcomes (Barnoski, 2003;Zazzali et al, 2008), and MDFT has been imported overseas to improve ASU services in clinics across western Europe (Rigter et al, 2013). Federal agencies have also sponsored large-scale dissemination of other evidencebased approaches such as A-CRA (Godley et al, 2011) and MET=CBT (Hunter et al, 2012), although federally funded initiatives present entirely distinct challenges regarding program sustainability.…”
