Objective Globally, 33 million cases of respiratory syncytial virus (RSV) infections occur annually among under-fives (5s). Ninety-nine percent of deaths from RSV occur in low- and middle-income countries. Under-five pneumonia mortality in Nigeria was estimated at 140,520 in 2017, but RSV epidemiological data are scant due to poor awareness and limited testing. Vaccines for RSV are currently under development and RSV mortality data from this high mortality, low resource setting are essential to maximizing the potential benefit of vaccination as well as promoting vaccine uptake. This study aimed to describe the epidemiology of RSV-associated acute lower respiratory tract infection (ALRTI) in children younger than 5 years in Zaria, Northwestern Nigeria. Methods A prospective cohort study was conducted among children aged 1 month to 5 years who were hospitalized with ALRTI in the Emergency Pediatric Unit of a tertiary hospital in Zaria from November 2018 to October 2019. Naso-pharyngeal swabs were obtained for RSV testing using a point-of-care immunoassay technique. Results Thirty-three percent (35/106) of the children had RSV-related ALRTI. The median age of RSV-positive cases was 8 months (IQR 3–14). Two-thirds of children (68.6%, 24/35) were below 12 months. The RSV mortality rate was 5.7% (2/35). RSV occurred in 10 months of the year with peaks in March and July. Conclusion A third of admitted children with ALRTI were positive for RSV. Therefore RSV significantly contributes to childhood pneumonia and the dual seasonal peak observed in our study may have implications for vaccine implementation.

Στην πρώτη επιδημιολογική μελέτη 604 ασθενών που επισκέφθηκαν τα ΤΕΠ μεγάλου τριτοβάθμιου νοσοκομείου ταυτοποίησαμε με μοριακές μεθόδους τα αίτια ιογενών λοιμώξεων του αναπνευστικού και εξετάσαμε κλινικο-επιδημιολογικές συσχετίσεις. Η μελέτη επικεντρώθηκε στις μικτές ιογενείς λοιμώξεις του αναπνευστικού για τις οποίες λίγα στοιχεία είναι δημοσιευμένα στην βιβλιογραφία. Ένας σημαντικός αριθμός μικτών ιογενών αναπνευστικών λοίμωξεων παρατηρήθηκε στην τρέχουσα μελέτη που αξιολόγησε τόσο τους ενήλικες όσο και τα παιδιά. Οι περισσότερες μικτές λοιμώξεις που παρατηρήθηκαν ήταν συλλοιμώξεις από τους ιούς RSV και γρίπης. Οι μικτές ιογενείς λοιμώξεις συσχετίζονταν με νεαρή ηλικία και αυξημένα ποσοστά πυρετού, αλλά δεν συνδέονταν με αυξημένα ποσοστά νοσηλείας. Δεν μπορέσαμε να συσχετίσουμε σε στατιστικά σημαντικό βαθμό την παρουσία μικτών ιογενών λοιμώξεων με υποκείμενες συν-νοσηρότητες. Ωστόσο, οι μικτές ιογενείς λοιμώξεις φάνηκε να εμφανίζονται συχνότερα από τις μεμονωμένες ιογενείς λοιμώξεις σε ασθενείς με σοβαρά υποκείμενα νοσήματα όπως ασθενείς με ΧΑΠ και ασθενείς με καρδιαγγειακή νόσο.Σε δεύτερη πιλοτική μελέτη με 76 ασθενείς περιγράψαμε, τις συσχετίσεις επιπέδων κυτταροκινών της οικογένειας της IL-17 με συγκεκριμένες ιογενείς λοιμώξεις του αναπνευστικού που τεκμηριώθηκαν με μοριακή μέθοδο. Αυξημένα επίπεδα κυρίων κυτταροκινών που συμμετέχουν στην Th17 απόκριση ανιχνεύθηκαν σε εργαστηριακά επιβεβαιωμένη γρίπη με τον τύπο A H3N2 σε σύγκριση με άλλους τύπους γρίπης, τον ιό RSV και άλλους αναπνευστικούς ιούς στην παρούσα μελέτη. Αυτή η εκσεσημασμένη Th17 απόκριση σε επίπεδο κυτταροκινών μπορεί να εμπλέκεται στην παθογένεση και τον ανοσιακό έλεγχο της οξείας λοίμωξης από τον ιό της γρίπης A H3N2. Παρόμοια ευρήματα για λοίμωξη με τους ιούς PIV είναι ενδεικτικά της συμμετοχής του μονοπατιού της Th17 ανοσιακής απόκρισης στον έλεγχο της λοίμωξης από τους ιούς της παρα-ινφλουέντσας. Μελλοντική εκμετάλλευση αυτού του μονοπατιού κυτταροκινών για διαγνωστικούς, θεραπευτικούς και προγνωστικούς σκοπούς μπορεί να βοηθήσει σε μεγάλο βαθμό στην διαχείριση των κοινών αλλά δυνητικά σοβαρών ιογενών λοιμώξεων του αναπνευστικού και είναι απαραίτητη.

With the recent SARS-CoV-2 outbreak, the importance of rapid and direct detection of respiratory disease viruses has been well recognized. The detection of these viruses with novel technologies is vital in timely prevention and treatment strategies for epidemics and pandemics. Respiratory viruses can be detected from saliva, swab samples, nasal fluid, and blood, and collected samples can be analyzed by various techniques. Conventional methods for virus detection are based on techniques relying on cell culture, antigen-antibody interactions, and nucleic acids. However, these methods require trained personnel as well as expensive equipment. Microfluidic technologies, on the other hand, are one of the most accurate and specific methods to directly detect respiratory tract viruses. During viral infections, the production of detectable amounts of relevant antibodies takes a few days to weeks, hampering the aim of prevention. Alternatively, nucleic acid–based methods can directly detect the virus-specific RNA or DNA region, even before the immune response. There are numerous methods to detect respiratory viruses, but direct detection techniques have higher specificity and sensitivity than other techniques. This review aims to summarize the methods and technologies developed for microfluidic-based direct detection of viruses that cause respiratory infection using different detection techniques. Microfluidics enables the use of minimal sample volumes and thereby leading to a time, cost, and labor effective operation. Microfluidic-based detection technologies provide affordable, portable, rapid, and sensitive analysis of intact virus or virus genetic material, which is very important in pandemic and epidemic events to control outbreaks with an effective diagnosis.
