Rapid onset severe thrombocytopenia following reexposure to piperacillin-tazobactam: report of two cases and review of the literature

Abstract: Pipercillin-tazobactam is a frequently used antibiotic that has a broad spectrum of antibacterial activity. The development of severe thrombocytopenia following the use of piperacillin-tazobactam is unusual. Several mechanisms have been proposed for the pathogenesis of thrombocytopenia in this setting which include immune and non-immune causes. Multiple case reports have shown the ability of piperacillin-tazobactam to cause drug-induced immune thrombocytopenia, likely through formation of antibodies that recog… Show more

“…Penicillin-like drugs, such as piperacillin, may trigger thrombocytopenia through this mechanism. Previous study has also shown that antibodies are recognised and bind to platelets in the presence of soluble piperacillin, thereby reducing platelet count 28. Elderly patients are more likely to have a rapid onset of immune thrombocytopenia due to repeated use of piperacillin-tazobactam 29.…”

“…In many cases, TMA, including cases of TTP, is detected at initial cancer diagnosis. 13,14 However, it may be seen at cancer recurrence and usually reflects the advanced stage of disease. 15 There were no signs of TTP with the initial diagnosis of malignancy in our patient.…”

“…Alzahrani et al reported two cases of possible immune thrombocytopenia that developed rapidly and severely in association with re-exposure to piperacillin-tazobactam. 15 Also the platelet count in peripheral blood improved after the discontinuation of piperacillin-tazobactam and the initiation of corticosteroid therapy, suggesting the possibility of immune-mediated thrombocytopenia.…”
