Rare case of acute dengue encephalitis with correlated MRI findings

Abstract: Dengue encephalitis is extremely rare, with most patients showing no significant abnormality on neuroimaging (CT/MRI). We report one of the very few documented cases of dengue encephalitis, with abnormal signal intensities on all major sequences on brain MRI.

“…31,3,35,41,49 More interestingly, bilateral thalami hyperintensities were seen in 14.3% of cases. 21,28,29,31,50 The "double doughnut" sign in which bilateral thalami T2 and FLAIR hyperintensity, and restricted diffusion with blooming artefact on gradient echo sequence in the centres of the thalami, were described in 2 cases. 29 One of the cases with double doughnut signs was associated with lesions in the cerebellum and another case had lesions in the pons and midbrain.…”

Mathew et al . described a 9‐year‐old with dengue encephalitis who had hyperintense T2WI signal at the thalami, pons, and midbrain, with corresponding T1WI hypointensity and restricted diffusion on DWI sequence.…”

“…Focal encephalitis-like changes were seen in 4 children in a series of 18 patients with dengue, as reported by Cam et al 40 They also looked at children diagnosed with dengue hemorrhagic fever who underwent cranial MRI and observed cerebral edema in most of the patients. Mathew et al 64 described a 9-year-old with dengue encephalitis who had hyperintense T2WI signal at the thalami, pons, and midbrain, with corresponding Figure 3 57-year-old lady who presented with headache, lethargy, and developed a single episode of generalized tonic-clonic seizure in the Emergency Department, which aborted spontaneously. She presented on day 10 of illness (she previously had recovered from "viral fever").…”
