Rate of Mesh Erosion After Sacrocolpopexy With Concurrent Supracervical Compared With Total Hysterectomy

Abstract: To estimate the effect of concomitant supracervical hysterectomy compared with total hysterectomy during abdominal sacrocolpopexy on the rate of mesh erosion by performing a systematic review and meta-analysis of the existing literature.DATA SOURCES: From database inception through January 2022, we explored MEDLINE, Web of Science, EMBASE, CINAHL, ClinicalTrials.gov, and Cochrane Central Register of Controlled Trials. Studies comparing the rate of mesh erosion in women undergoing abdominal sacrocolpopexy who h… Show more

“…A patient with a history of abnormal Pap tests or elevated risk of cervical malignancy (such as HIV or high-risk human papillomavirus infection) or abnormal uterine bleeding concerning for or increased risk of endometrial hyperplasia/malignancy may choose to have her uterus completely removed. Those without a history of cervical disease may benefit by keeping the cervix in place, as this helps to reduce mesh wrinkling and is associated with lower rates of mesh exposure 2 …”

“…In the last decades, the FDA, starting from the literature data, have unquestionably stated that opening the vagina during prosthetic prolapse reconstruction surgery should be avoided and, in particular during sacral colpopexy, the risk of erosion statistically increases in cases of vaginal incision-opening for total hysterectomy [16].…”

“…In the largest observational cohort study by van Zanten et al [ 18 ], mesh exposure rates were similar for both robotic procedures (2.1% for TH vs. 0.9% for SCH; P >0.999). A most recent systemic analysis review of 19 studies for rate of mesh erosion demonstrated that SCH had a lower rate of mesh erosion than that in TH (combined odds ratio [OR], 0.26; 95% confidence interval [CI], 0.18–0.38; I 2 , 0%) [ 21 ]. This review suggested that this may be due to the opening of the vaginal cuff weakening the tissue because of vaginal bacteria, tissue devascularization, or loss of structural support of the cervix for mesh or suture attachment.…”
