Rationale and design of the INNOVATE Trial: an international cooperative study on surgical versus conservative treatment for odontoid fractures in the elderly

Abstract: BackgroundFractures of the odontoid process of the axis are the most common fractures of the geriatric cervical spine. As the population ages, their incidence is expected to increase progressively, as is the number of very old patients (>80 years) with an odontoid fracture. No consensus exists on the optimal treatment (surgical or conservative) and the most relevant outcome parameter (osseous union, fracture stability or clinical outcome). The aim of the INNOVATE (INterNational study on Odontoid frActure Treat… Show more

“…• • There are ongoing RCTs and multicenter cohort study (not population-based) on OFx treatment that we strongly support [112][113][114]. However, because of reasons previously mentioned, population-based cohort studies can provide significant information when all OFx patients from a general population are included.…”

“…The medical complexity of OFx patients is probably one of the reasons why we lack Class I evidence supporting treatment decisions [7]. There are, however, ongoing studies which we hope will provide increased knowledge [27,28]. Since a large fraction of these patients are too old and frail to tolerate general anesthesia and open surgery within acceptable risk, randomized controlled trials (RCTs) to compare surgical versus conservative treatment would be biased by the need to exclude a major portion of the patients in question.…”

“…In older patients, the achievement of this balance is even more challenging. [12] Therefore, the objective of this meta-analysis is to summarize and compare the outcomes of surgical and conservative treatments for type II and type III odontoid fractures in the elderly, focusing primarily on the nonunion rate and mortality rate, and secondarily on patient satisfaction, complications, and the hospital stay.…”
