Raynaud's phenomenon of the nipple as a side-effect of labetalol: Case report and literature review

Abstract: Raynaud's phenomenon of the nipple is an unusual side-effect of labetalol use. There is one official report from the United Kingdom. The present case concerns a 30-year-old woman, gravida 1, para 0, who developed pre-eclampsia and was treated with labetalol but subsequently reported neuropathic pain of the nipple. Nifedipine was then started as part of her treatment plan for blood pressure control and she no longer reported pain, despite being given six more doses of labetalol. “…Raynaud phenomenon (RP) is characterized by vasospasm of arterioles causing intermittent ischemia and reflex vasodilatation. Vasospasm is a physiological response to cold temperatures but can also be secondary to stress, caffeine consumption, and smoking 3 . “…RPN particularly affects pregnant and lactating patients, with risk of breastfeeding cessation due to pain 8 . Though primarily idiopathic, drug‐associated RPN with labetalol has been reported 3,8 . Also, literature has linked breast implants and autoimmune disorders including Raynaud's, and a case series of 12 lactating mothers suggested an association between history of breast surgery and RPN.…”

“…Certain medications, including drugs for migraine, cyclosporine, and nonselective beta-blockers, can also lead to secondary RP [ 31 ]. RP of the nipple has also been reported to occur following the administration of labetalol, a first-line drug employed for gestational hypertension [ 105 ], with the symptoms resolving on discontinuation of this drug.…”

“…Важно отметить, что во всех представленных случаях возникновение похолодания сосков совпало по времени с максимальной концентрацией лабеталола в плазме крови беременных женщин [35]. Единичные случаи развития синдрома Рейно сосков при применении лабеталола были официально зарегистрированы в Соединенном Королевстве и США [36,37].…”
