RD Rio Mycobacterium tuberculosis Infection Is Associated with a Higher Frequency of Cavitary Pulmonary Disease

Abstract: Molecular genotyping has shown Mycobacterium tuberculosis lineages to be geographically restricted and associated with distinct ethnic populations. Whether tuberculosis (TB) caused by some M. tuberculosis lineages can present with a differential clinical spectrum is controversial because of very limited clinical data. We recently reported on the discovery of RD Rio M. tuberculosis, a Latin American-Mediterranean sublineage that is the predominant cause of TB in Rio de Janeiro, Brazil. To investigate the clinic… Show more

“…The high transmissibility of RD Rio strains is also suggested by studies showing an elevated secondary case rate in San Francisco, CA (38), and New York, NY (24), while a higher active-TB case rate in persons infected by RD Rio strains was described in The Gambia (30). In Brazil, strains carrying RD Rio have been isolated from one-third of the TB cases from two geographically distant metropolises (21,22), a rate similar to that observed in the present study. Notably, in all of them RD Rio was the main agent responsible for causing TB cases among all other spoligofamilies.…”

“…One possible strategy used by RD Rio is to induce more cavitation, as was previously demonstrated (22), which is a known risk factor for transmissibility. In the current study, however, we were unable to show an association with cavitation, and this is probably related to population selection, as the former study involved TB patients with suspected drug resistance or comorbidities.…”

“…), rare stranger. While the LAM-RUS branch was studied only by one Russian group, the RD-Rio sublineage has attracted much more attention since its discovery (5)(6)(7)29), but evidence of its increased virulence and transmissibility appears controversial (30,31). In this study, only a few isolates belonged to RD-Rio (7 such isolates) (Fig.…”
