Re-inventing adherence: toward a patient-centered model of care for drug-resistant tuberculosis and HIV

Abstract: SUMMARY BACKGROUND Despite renewed focus on molecular tuberculosis (TB) diagnostics and new antimycobacterial agents, treatment outcomes for patients co-infected with drug-resistant TB and human immunodeficiency virus (HIV) remain dismal, in part due to lack of focus on medication adherence as part of a patient-centered continuum of care. OBJECTIVE To review current barriers to drug-resistant TB-HIV treatment and propose an alternative model to conventional approaches to treatment support. DISCUSSION Curr… Show more

“…In South Africa where HIV diagnostic testing and capacity for drug-susceptibility testing of MTB isolates is robust, 40-80% of patients with XDR-TB are HIV infected [23,24]. In the Russian Federation and Eastern Europe, XDR-TB and HIV are often associated with injection drug use [25].…”

“…For example, there are highly efficacious drugs available to treat TB (i.e., they work very well under optimized conditions). However, despite this, there are many impediments to TB elimination which are predominately “operational,” i.e., the real-world effectiveness of these efficacious drugs is reduced by a range of multi-level barriers (38, 39). Similarly, there are also now highly efficacious drugs for HCV offering the potential for a cure; however, here, the real world effectiveness of these agents is undercut by issues of cost and resultant processes that restrict access (40, 41).…”

“…3 Patient-centered care that incorporates counseling and support is increasingly being recognized as an approach to improve outcomes. [4][5][6] Social workers, who are often staff at hospitals and clinics that treat DR-TB patients, may be an overlooked resource.…”
