Readiness for transition to adult health care among US adolescents, 2016–2020

Abstract: Objective Adolescence is a critical period of transition from paediatric to adult health care, but readiness for this transition has been described as low in the general adolescent population. “…All patients underwent comprehensive history taking, physical examinations, and a battery of audiological examinations. The age of onset of hearing loss was collected for each patient and they were divided into five groups: prelingual (below age 4 years), preschool (aged between 4 and 6 years), early school to teenage (aged between 6 and 18 years), young adult (aged between 18 and 40 years), and late-onset (aged > 40 years) [ 14 , 18 , 19 , 20 ]. The battery of audiological examinations included tympanometry, sound field audiometry, pure-tone audiometry, otoacoustic emission tests, auditory brainstem response tests, and auditory steady-state response tests.…”

“…In this phase, patients and clinicians should agree on a timeline, including establishing criteria to detect transition-ready patients and regularly conducting readiness assessments. Tracking readiness over a 5-year period can effectively show when adolescents meet criteria for transitioning [28]. At around 17 years of age, joint meetings should be foreseen to ensure a successful transition by age 18.…”

“…This developmental change coincides with the transition from pediatric to adult primary care, which should ideally occur when the patient is between the ages of 18-21 [ 2 ]. Adolescent readiness to transition to adult health care has increased somewhat in recent years, but remains low [ 3 ].…”
