Real-Time 3D Echocardiographic Quantification of Left Atrial Volume

Abstract: Compared with CMR reference, 3DE-derived LAV measurements are more accurate than 2DE-based analysis, resulting in fewer patients with undetected atrial enlargement.

“…366,367 The advent of 3-D ECHO has improved the accuracy of ECHO volume measurements which correlate well with cardiac computed tomography 368,369 and magnetic resonance imaging. 370,371 Compared with 2D assessment of LA volume, 3DE also has superior prognostic prediction. 372,373 The recommended upper normal indexed LA volume is 34 mL/m 2 for both genders which fits well with a risk-based approach for determination of cut-off between a normal and an enlarged LA.…”

“…Volumetric data from cardiac computed tomography (CCT) are comparable to data generated by CMR and 3D echocardiographic imaging and is superior to 2D echocardiography. 371 The LA volume prior to catheter ablation and the presence of asymmetry of chamber geometry predicts the likelihood of maintaining sinus rhythm post-procedure. 408 As the LA enlarges, the shape of the LA roof initially becomes flat and then becomes coved, and this progression may correlate with development of non-PV substrate in patients undergoing AF ablation.…”

“…Echocardiographic 3D data sets are usually obtained from the apical approach using a multi-beat full volume acquisition. (22,23) This algorithm is time consuming, needs postprocessing and probably underestimates true left atrial volume in comparison with the gold standard magnetic resonance imaging (CMR). (24) Left atrial size and volume can be evaluated with computerised tomography (CT), usually before radiofrequency ablation of atrial fibrillation, (25) and before closure of left atrial appendage.…”

“…Three-dimensional (3D) echocardiographic left atrial volume calculation shows better correlation with cCT and magnetic resonance compared to 2D volumes (53,54). 3D volume analysis does not require any geometric assumption.…”
