Reassessment of Lhermitte’s sign in multiple sclerosis

Abstract: The reliability and diagnostic value of Lhermitte's sign in multiple sclerosis (MS) has not been fully established. The purpose of this study was to determine the clinical, neurophysiological and neuroradiological correlations of Lhermitte's sign in a cohort of MS patients and reassess the relevance of this phenomenon in the clinical history of the disease. A prospective study of 694 patients with MS and 110 age-matched healthy adults was evaluated by a structured questionnaire that included basic demographic … Show more

“…40-80% of the patients claim that paraesthesia symptoms (tingling) affect their daily life, especially when they become persistent [56,62]. Furthermore, some people with MS may experience dysaesthesia and Lhermitte's phenomenon as a result of spinal cord demyelination [63,64]. Dysaesthesia is manifested as an intense pain of pins and needles and burning sensation, usually present in the extremities [58,65].…”

“…Magnetic resonance imaging (MRI) of the cervical spine is indicated to rule out spinal cord injury. Demyelination is more likely to be detected in multiple sclerosis patients with Lhermitte's sign than those without [22]. On the other hand, no MRI abnormalities were noted in a study of 5 OLS patients [12].…”

“…2022 However, a more recent prospective study of 694 multiple sclerosis patients found that about 16% of the patient cohort experienced Lhermitte’s sign and that this was significantly correlated with neuroradiological and neurophysiological abnormalities. 23 While further studies are still needed to critically assess Lhermitte’s sign, its presence in other spinal and radicular pathologies reinforces that enquiring about Lhermitte’s sign during neurological examination should be a mainstay of clinical practice. 22…”
