Reassessment of the Assumed A-V Oxygen Content Difference in the Shunt Calculation

Abstract: A study was undertaken in 15 patients to compare measured and assumed arteriovenous oxygen (A-V O2) content differences and their effects on resultant shunt calculations. All patients were on volume ventilators and demonstrated a stable cardiovascular state. Simultaneous measurements of the O2 content of a pulmonary artery (PA) and of a superior vena cava (SVC) sample were compared. A mean A-V 02 content difference of 3.5 plus or minus 0.8 volumes percent was obtained from the PA and 2.6 plus or minus 1.1 volu… Show more

“…Pure oxygen ventilation for PaO 2 /Fio 2 measurement may reduce the impairment of the correlation between lnPaO 2 / Fio 2 and physiological shunt or CT shunt by regions with low ventilation-to-perfusion ratios as previously pointed out by others and reproduced in our study (5,12,18,19,27,35,37,47,48,(67)(68)(69). Additionally, pure oxygen ventilation may blunt hypoxic pulmonary vasoconstriction which may increase the sensitivity of lnPaO 2 /Fio 2 for assessing CT shunt (26,69).…”

“…It refers to the venous admixture calculated by standard formulae using measured oxygen contents of mixed venous and arterial blood, during 100% oxygen ventilation, in order to eliminate the influence of poorly ventilated areas (18,33,37,38,(46)(47)(48)(49)(50).…”

“…7 We also assumed the PAoz tO be proportional to Flu2 as we did when estimating A-aDo2. The oxygen saturation, which in this case is not directly measured, was obtained from the homogram for oxygen-haemoglobin dissociation curve (Severinghauz).…”
