Recent Progress in Radiofrequency Ablation Therapy for Hepatocellular Carcinoma

Abstract: In order to attain better ablation and more effective management of hepatocellular carcinoma (HCC), new approaches and devices in radiofrequency ablation (RFA) therapy were presented and discussed in a workshop at the 50th Annual Meeting of the Liver Cancer Study Group of Japan. A novel bipolar RFA apparatus was introduced in Japan in January 2013. Hundreds of subjects with HCC were treated with multipolar RFA with varied devices and plans. Among these, no-touch ablation was one of the most useful procedures i… Show more

“…In conventional RFA of other organs such as the liver, the active tip of the electrode is fixed to the center of the target tumor, which is ablated by the conducted heat; the ablation zone resulting from the fixed electrode technique is round in shape (23). However, as thyroid nodules are often ellipsoidal exophytic from the thyroid gland, they are difficult to ablate using the fixed technique.…”

“…Bipolar RFA has been used in liver, kidney, and muscle treatments (23). Unlike monopolar electrodes, use of bipolar electrodes simplifies the procedure, as grounding pads are not required.…”

“…This approach often can be applied through open surgical, laparoscopic, or percutaneous techniques. RFA can target the tumor within the parenchyma through image guidance and spare the normal surrounding liver [8]. Treatment outcomes indicate that RFA is a safe and efficient method of tumor ablation with low morbidity and mortality rates, and maximal perseveration of normal liver parenchyma [9].…”

“…No ground pad is required, so it does not cause thermal injury to the skin [9], and the system can also be used safely in patients with cardiac pacemakers [18]. One of the greatest advantages of the multipolar system is its ability to achieve a large ablative area in a short period of time as planned, because it is not prone to a vascular heat sink effect that would result in unexpected reduction of the ablative area [19,20].…”
